' Phase IVb Weekly Summary No. 16 Pale 3 003-2203
approximately 42,304 square feet in the Phase IVb OSA during this reporting period. Geomembrane
panels P-603 through P-6li were deployed, seamed, and pressure tested by Poly-Flex. Destructive
fusion seam samples DF-164 through DF-168 and extrusion seam sample DX-29 were marked and
removed. Destructive fusion and extrusion seam peel and sheaz tests of DF-164 through DF-168 and
DX-29 were performed and passed per project specifications.
Phase IVc
Ames resumed limited cut to fill and cut to temporary stockpiling activities within the limits of the
Phase 1Vc area. Ames hauled Structural Fill (SF) material from the Phase IVc azea to temporary
stockpiles on Chicken Hawk Hill and in the Saddle Fill.
Field Monitoring and Testing Activities
Golder observed construction activities associated with Phase N earthworks. All construction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications. Removal of oversized or deleterious material from all fill
azeas was observed. Golder observed Structural Fill (SF) placement in Phase IV for materials, lift
thickness, moisture content, and compactive effort.
Golder observed placement, compaction, and conditioning of LVSCF in the Phase IVb PSSA sump
for subgrade preparation of the High Volume Solution Collection System (HVSCS) riser
foundations. Golder CQA monitors performed four nuclear moisture-density tests (LVSCNb-O1
through LVSCNb-04) on LVSCF placed within the Phase IVb PSSA sump. The tests performed met
the requirements of the specifications and the nuclear moisture-density results summary is presented
in Attachment B.
Golder visually observed the placement of LVSCF in the Phase Nb PSSA area. Golder CQA
monitors (one monitor per dozer when dozers are greater than 50 ft apart) observed LVSCF
placement.
Golder observed anchor trench excavation in the Phase IVb PSSA and OSA for proper location and
dimension (2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the
specifications. Golder observed anchor trench backfill in the Phase IVb PSSA for lift thickness and
compaction.
Golder observed construction of the Primary Underdrain in Phase IVb for trench dimension (5' wide
by 3' deep), slope (minimum 1%), materials, and geotextile overlap.
Golder observed and monitored placement of Bedding Fill (BF) in the Phase IVb PSSA for proper
location and thickness (1' deep). Golder monitored compactive effort and fmish grading to verify
conditions met specification.
Golder observed geomembrane deployment, documenting ambient air temperature, panel
dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and
destructive). Geomembrane placed in the Phase IVb area met the project specifications.
Phaze N616 Mayo-May10, 200i.doc Golder Associates lnc.
