Soil Liner Fill placement was performed in accordance with the Phase N VLF Construction <br />' Specifications. Results and locations of field density and depth verification testing aze presented in <br />Tables 7 and 8, respectively. <br />' Construction of the containment liner extension required exposing the original Phase I liner anchor <br />trench, and excavation of approximately 980 feet of new anchor trench along the southern edge. <br />Golder observed and documented the deployment, seaming, defect repair, and testing (Destructive <br />and Non-Destructive) of approximately 37,786 square feet of 100-mil single-sided textured and <br />100-mil smooth LLDPE geomembrane deployed by Poly-flex. The as-built geomembrane panel <br />layout is presented in Figure 2. Geomembrane was supplied and tested in accordance with the Phase <br />' N VLF Construction Specifications and was placed to complete the composite liner system. CQA <br />observation records and test results for the installation of geomembrane are presented in <br />Attachment B. <br />Golder visually monitored the placement of approximately 2,800 cubic yards of DCF over the <br />geomembrane adjacent to the ADR and the lab. DCF was not placed west of the lab area, Golder <br />observed that DCF was placed to maintain a thickness of 2 to 3 feet of DCF in the traffic azea next to <br />' the ADR. <br />Other information required by the Phase N VLF Construction Specifications for the placement of <br />SLF and geomembrane, including soil surface acceptance certification, geomembrane certification <br />summazies, equipment calibration certificates, and resumes of Poly-flex personnel, are presented in <br />Attachment C. <br />The containment liner extension was constructed in accordance and met with the Phase N VLF <br />- Construction Specifications, with the following modifications: <br />• The minimum constructed liner overlap between the existing VLF liner and the <br />containment liner extension was 1 foot. This was considered acceptable because <br />this seam will not be subjected to high ore loads and ore leaching will not occur in <br />this area; and <br />• A maximum slope of 2 horizontal (I-I):1 vertical (V) was used in some areas of the <br />I containment liner extension azea to facilitate construction in a limited space.
