CO USA 80228 <br />Telephone (303) 980-0540 <br />Fax (303) 985-2080 <br />www.goldeccom ' <br />December 10, 2003 Our Ref: 003-2203 <br />' Cripple Creek & Victor Gold Mining Company <br />1280 State Highway 67 <br />Victor, Colorado 80860 <br />Attention: Mr. Ron Roberts <br />RE: 2003 CONTAINMENT LINER EXTENSION, CONSTRUCTION QUALITY <br />ASSURANCE (CQA) MONITORING <br />Dear Ron: <br />Golder Associates Inc (Golder) has prepared this report for the Cripple Creek & V ictor Gold Mining <br />Company (CC&V) presenting the Construction Quality Assurance (CQA) observations and data <br />completed for the containment liner extension neaz the ADR facility. <br />The containment liner extension encompasses an area of approximately 32,000 square feet (based on <br />grading plan) along the southern boundary of the Phase I Valley Leach Facility (VLF). This area was <br />t graded and lined to provide hydraulic containment for the ADR facility. <br />Description of Activities <br />Construction activities were performed by Ames Construction Ina (Ames) and Poly-flex <br />Construction Inc. (Poly-flex) during the period of August 18, 2003 through October 24, 2003. Golder <br />observed and documented the construction activities including subgrade preparation, placement of <br />Structural Fill (SF), Soil Liner Fill (SLF) placement, Pipe Bedding Material (PBM) placement, <br />anchor trench excavation, geomembrane deployment, and Drain Cover Fill (DCF) placement. <br />As part of the containment liner extension, a Phase I leak detection sump (LDSl) was relocated and <br />the leak detection trench was extended approximately 290 feet to the west-southwest. Figure 1 <br />illustrates the final top of liner grading within the containment liner extension area, and includes the <br />' new location of the LDSI. <br />Golder conducted laboratory testing as required by the Phase N VLF Construction Specifications for <br />DCF, SF, PBM, and SLF. Tables 1 and 2 present a summary of the Technical Specifications and testing <br />frequency. The laboratory test results are summarized in Tables 3 through 6 and presented in <br />Attachment A. Attachment B presents geomembrane observations and test results (including ambient <br />temperatures). Attachment C presents subgrade acceptance forms, geomembrane and resin certifications, <br />equipment calibrations, Golder staff schedule, and Poly-Flex resumes. <br />Field material testing was conducted by Golder on SLF placed in the ADR area. The field testing of <br />approximately 1,400 cubic yards of SLF included monitoring of placement and preparation for <br />geomembrane deployment, testing in-place moisture and density, and performing depth verification <br />r10iS1i <br />• <br />OFFICES ACROSS AFRICA.
