Phase II-III Summary No. 2 Page 2 043-2170 <br />reworked, retested, and maintained prior to SLF surface acceptance. <br />Ames completed excavation of the Phase II-III Extension anchor trench. Ames excavated <br />approximately 550 feet of anchor trench in preparation for geomembrane insta11a6on during this <br />reporting period. <br />Ames continued production of Drain Cover Fill (DCF) material at the screen plant by the secondary <br />crusher. <br />Poly-Flex commenced and completed deployment of 80-mil single-sided textured LLDPE <br />geomembrane for the Phase II-III Extension construction. Geomembrane was installed in the Phase II- <br />III Extension on the east facing slope of Beacon Hill downwazd to the Phase IUIII tie-in. Poly-Flex <br />deployed 80-mil single-sided textured LLDPE geomembrane over an azea of approximately 75,565 <br />squaze feet in the Phase II-III extension during this reporting period. Geomembrane panels X-1 through <br />X-42 were deployed, seamed, and tested by Poly-Flex. Destructive fusion and extrusion seam samples <br />DFx-Ol through DFx-09, and DXx-O1 through DXx-02 were marked and removed. Destructive fusion <br />and extrusion seam peel and sheaz testing on samples DFx-Ol through DFx-09, and DXx-Ol through <br />DXx-02 were performed and passed per project specification. <br />A map of the azea worked during this reporting period is provided in Attachment C. <br />Field Monitoring and Testine Activities <br />Golder observed construction activities associated with Phase II-III Extension earthworks. All <br />construction materials and soil materials placed were verified as suitable, and construction methods <br />were in accordance with project specifications. <br />Golder observed excavation of the Phase II-III Extension anchor trench for proper location, dimension <br />(2' wide by 3' deep) and maintenance prior to geomembrane deployment. Anchor trench excavation <br />met or exceeded the requirements of the specification. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase II-III extension <br />for subgrade prepaza6on, thickness, compaction, moisture-content, and materials used. The Golder <br />CQA Earthworks Monitor performed six nucleaz moisure-density tests (SLNx-1 through SLNx-6) and <br />performed two nucleaz moisture-density retests (SLNx-la and SLNx-Sa). Field test results are <br />presented in Attachment B. <br />Golder observed geomembrane deployment, documenting ambient temperature, panel dimensions, <br />thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure testing).
