Grand Gravel Pit Conversion to 112 Adequacy Review <br />M-1998-019 <br />July 5, 2005 <br />Page 2 <br />4. Rule 6.4.3(c) requires that Exhibit C include one or more maps to legibly portray the existing <br />topography of the area with contour lines of sufficient detail to portray the direction and rate of slope <br />of the affected land. Please submit a new or revised map depicting the topography. <br />5. County Road 2 and the boundary fences appear to be within 200 feet of the affected boundaries <br />shown on Exhibit C, but were not described as structures as required by Rule 6.4.3(g). Please <br />provide the names and addresses for the owners of all structures within 200 feet of the affected <br />lands, and assure the Division that notice of the application was provided to these owners of <br />structures. Please be aware that Rule1.1(48) defines "Structure, Significant, Valuable and <br />Permanent Man-made" to include, but not be limited to, "buildings, houses, barns, fences, above or <br />below ground utilities, irrigation ditches, maintained or public roads, bridges, railroad tracks, <br />cemeteries, communication antennas, pipelines, water welts, water storage structures, discharge and <br />conveyance structures, etc." <br />Rule 6.4.4 Exhibit D -Mining Plan <br />6. Per Rule 6.4.4(c), please describe all water diversions and impoundments sufficiently to bond for <br />their removal. Please describe the handling of any wastewater generated on the site (e.g., scrubber <br />blow down water, concrete truck wash water). Please also describe any storm water run-off controls <br />that may be implemented. <br />7. To assist in the calculation of the financial warranty and as required by Rule 6.4.4(d), please indicate <br />the size of areas to be worked at any one time. Please indicate the maximum area of disturbance, <br />linear feet of any highwall, the grade of pit side slopes to be maintained during mining, and maximum <br />size of area to be topsoiled. Please relate the approximate timetable requested by Rule 6.4.4(e) to <br />the extent of mining disturbance and the process of reclamation. <br />8. The application states that Grand Gravel expects contractors to install an asphalt plant and concrete <br />batch plant on site. To ensure the adequacy of the calculated bond, the Division needs the applicant <br />to describe what controls will be implemented to ensure materials associated with asphalt production <br />and distribution do not adversely impact the environment. <br />• Please locate the structures and associated environmental controls on the Mining Plan map. <br />• Please list any chemicals and/or hazardous waste, such as asphalt cement and diesel fuel, that <br />will be used on site in association with these plants, the volumes of each, their storage locations, <br />and how they will be stored. <br />• Please describe how the facilities will be managed to ensure groundwater and surface water will <br />not be impacted. Please describe what controls will be implemented to ensure materials <br />associated with asphalt production and distribution (such as oil unloading and storage facilities) <br />do not adversely impact the environment. What actions will be taken the event of a leak or spill? <br />9. What is the maximum amount of waste asphalt that will be stored on site at any one time? Please <br />locate the stockpile on the Mining Plan map. What environmental controls will be used in waste <br />asphalt storage and/or disposal? <br />10. What does it mean that the "portable mobile concrete batch plant will have own water source?" <br />Please describe the potential source(s) of the water, keeping in mind that all water used in the <br />operation must be decreed and/or permitted for such use. <br />11. Regarding water to be used by the operation (e.g., dust control, in the batch plants), it is my <br />understanding, from discussions with Mr. Richard Curry during the June 27, 2005, inspection, that all <br />water used in the operation is diverted from the river and stored during "free river" (no call) <br />
