Curry: <br />This letter lists the Division's adequacy review comments on the Grand Gravel Pit Conversion <br />Application, Revision CN-01 for Permit Number M-1998-019. I apologize for the delay in providing this to <br />you. The Division of Minerals and Geology (Division) is required to make an approval or denial <br />recommendation no later than July 12, 2005, therefore, your response to the following adequacy review <br />concerns should be submitted to the Division as soon as possible. <br />Since all but 1.57 acres of the 19.37 acres that are the subject of this 112 conversion application are <br />already permitted as the 9.8-acre 110 Permit No. M-1998-019 issued to Grand Gravel and the 8-acre <br />111 Permit No. M-2005-024 issued to Lafarge, please provide a letter or letters signed and notarized <br />by both Grand Gravel and Lafarge stating the agreed upon relationship between the permits upon <br />approval of the 112 permit. For example, if the operators prefer that the Division terminate the permit <br />held by Lafarge upon approval of the 112 permit and release Lafarge from reclamation responsibility, <br />please clearly state this in the letter. Without such a letter, the Division will need to hold both <br />operators responsible (and both associated bonds) for reclamation of the 8-acre portion of the parcel <br />until reclaimed. <br />2. Per Rule 6.2.1(2), General Requirements -Maps and Exhibits, all maps (other than the <br />index/location map) must show the name of the Applicant; must be prepared and signed by a <br />registered land surveyor, professional engineer, or other qualified person; give the date prepared; <br />identify and outline the area which corresponds to the application; and show a map scale, legend, <br />title, and a north arrow. Please submit revised Mining Plan and Reclamation Plan maps that <br />incorporate all of these items. On these maps, please also tie the pit site location to a known point, <br />such as providing the distance and direction to a section corner; the 1/16' section corner apparently <br />was cut off from the Mining Plan and Reclamation Plan maps when they were enlarged to add detail. <br />Rule 6.4.3 Exhibit G -Pre-Mining and Mining Plan MaD(sl of Affected Lands <br />3. Rule 6.4.3(b} requires identifying various structures located on the affected land and within 200 feet <br />of its boundaries, however, it appears that the maps submitted do not depict areas within 200 feet of <br />the boundaries on all sides.
