Phase IVb Weekly Summary No. 30 Page 3 003-2203 <br />Poly-Flex began deployment of 100-mil single-sided textured and smooth LLDPE geomembrane in the <br />ADR Liner Expansion area Poly-Flex deployed 100-mil single-sided textured and smooth LLDPE <br />geomembrane over an area of approximately 18,526 square feet in the ADR liner expansion azea during <br />ties reporting period. Geomembrane panels X-1 through X-25 were deployed, seamed, and tested by <br />Poly-Flex. Destmctive fusion and extension seam samples DFx-Ol through DFx-02 and DXx-Ol <br />through DXx-02 were marked and removed. The results of testing on destnrctive fusion and extrusion <br />samples collected during this reporting period passed per project specification. <br />Ames continued to remove DCF in areas north and northwest of the CC&V lab in the ADR liner <br />expansion area in preparation for geomembrane deployment. <br />Field Monitorine and Testing Activities <br />Go]der observed construction activities associated with Phase IV earthworks. All construction materials <br />and soil materials placed were verified as suitable, and construction methods were in accordance with <br />project specifications. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA <br />and the ADR Liner Expansion area for subgrade prepazation, thickness, compaction, moisture <br />content, and materials. The Golder CQA Earthworks monitor performed fourteen nuclear moisture- <br />density tests (SLNb-223 through SLNb-233, and SLNx-O1 through SLNx-03) and performed twelve <br />nuclear moisture-density retests on SLF that required reworking (SLNb-226a, 2296, 232a, 2326, <br />232c, and SLNx-Ole, SLNx-Olb, SLNx-Olc, SLNx-02a, SLNx-026, SLNx-03a, SLNx-036) located <br />within the Phase IVb OSA (designated by suffix `b') or ADR Liner Expansion area (designated by <br />suffix `x'). The SLF met the requirements of the specifications prior to deployment of <br />geomembrane. Field test results are presented in Attachment B. <br />Golder observed placement of secondary and tertiary HVSCS piping in the Phase IVb OSA, <br />verifying construction and layout per the project plans. <br />Golder visually observed placement and grading of DCF in the Phase IVb OSA. Golder CQA <br />monitors (one monitor per dozer when dozers are greater than 50 ft apart) observed DCF placement <br />and verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations, <br />5 feet for 773 equipment, and 10 feet for haul road construction aad equipment. <br />Golder visually observed removal of DCF from the Phase IVb OSA /Phase III tie-in area and Phase <br />I / ADR liner expansion tie-in area. Golder mazked damage caused to the geomembrane for <br />subsequent repair during ADR geomembrane tie-in construction. <br />Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. <br />Golder monitored Leak Detection Trench excavation and construction in the Phase IVb OSA for <br />proper location and dimension (1' wide by 1' deep), slope (1% minimum), and materials used <br />Pratt N630 AVgu5Ui-August30, 2003 Golder Associates Inc. <br />
