• over an area of 64,048 squaze feet, and deployed 100-mil DST LLDPE geomembrane over an area of
16,246 squaze feet in Phase IVb during this reporting period. Geomembrane panels L363 through L385,
P386 through P395, L396 through L437, and M438 through M466 were deployed, seamed, and tested
by Poly-Flex. Destructive fusion seam samples DF-99 through DF-129 were mazked, removed, and
tested. Destructive extrusion seam samples DX-10 through DX-13 were marked, removed, and tested.
Ames continued placement of Structural Fill (SF) in the Phase IVb azea. Ames placed SF above the
Phase IVb sump. Ames also continued scope recontouring in the Phase Nb area on Substation Hill, on
the PSSA floor, and on the Phase III ore slope.
Ames conducted SF blasts in Phase IVb near station 14+00 of the permanent diversion channel on
October 16~' and 25's at 17:00 and 12:15, respectively.
Ames continued placement of Bedding Fill (BF) over the existing Phase III ore slope in preparation for
geomembrane deployment. Ames placed one foot of BF from the edge of Phase III liner to the 9606
bench.
Ames continued excavation of the interior anchor trench along the 9606 road on the Phase III ore
slope in preparation for geomembrane deployment. Ames excavated approximately 500 linear feet
of anchor trench.
Ames resumed construction of Phase IVb Leak Detection Trench (LDT) starting near the PSSA East
• end berm and working South. Ames excavated approximately 300 feet of LDT.
Ames resumed placement of Drain Cover Fill (DCF) at the PSSA West end berm. Ames placed two
feet of DCF over previously placed 80-mil LLDPE geomembrane in preparation for end berm
construction.
Phase IVc
Ames performed excavation remediation activities for underground workings UG-291 and UG-498.
These workings were observed and verified to be "Deep" as defined in the drawings and will be
remediated accordingly.
Ames performed confirmatory drilling activities associated with the Phase IVc construction area on
underground workings UG-308, UG-309, and UG-310.
Ames placed Cemented Rockfill (CR) at UG-270 and UG-292 by tapering the workings and placing 7
feet of CR in accordance with IFC Drawing 12, Detail 1.
Ames resumed placement of Coarse Shaft Backfill (CSB) into "Deep" workings at UG-500, UG-502,
UG-503, and UG-504. CSB was placed in prepazation for Concrete Plug construction.
