Destmctive fusion and extrusion seam <br />samples DFb-31 through DFb-33 and DXb-09 through DXb-11 were marked and removed. Destructive <br />fusion and extrusion seam peel and shear tests on 100-mil smooth LLDPE geomembrane deployed <br />during this reporting period passed per project specifications. <br />A map of the azea worked during this reporting period is provided in Attachment C. <br />Phase IVc <br />Ames performed no construction activities in the Phase IVc area during this reporting period. <br />Field Monitoring and Testine Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed the placement of secondary and tertiary HVSCS piping in the Phase IVb PSSA and <br />Phase IVb OSA, verifying construction per the project plans. <br />Golder visually observed the placement of DCF in the Phase IVb PSSA and OSA areas. Golder <br />CQA monitors (one monitor per dozer when dozers are greater than 50 ft apart) observed DCF <br />placement and verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe <br />locations, 5 Feet for 773 equipment, and 10 feet for haul road construction and equipment. <br />Golder visually observed the placement of SDCF and ore in the vicinity of the Phase IVb vertical <br />risers on a part-time basis. Golder visually observed construction of the HVSCS vertical risers in the <br />Phase IVb sump on a part-time basis. <br />Golder visually observed removal of DCF from the Phase IVa anchor trench and the Phase III toe at <br />Phase IVb tie-in areas. Damage caused to geomembrane was marked for subsequent repair. <br />Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2' wide by 3' deep), Anchor trench excavation met or exceeded the requirements of the <br />specifications. <br />Golder observed underground working excavations in the Phase IVb OSA on the south side of <br />Substation Hill. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA <br />for subgrade preparation, thickness, compaction, moisture content, and materials. Golder CQA <br />monitor performed 27 nuclear moisture-density tests (SLNb-129 through SLNb-153), and 10 depth <br />verification checks (SLDb-86 through SLDb-95) on SLF placed within the Phase IVb OSA. Two of <br />the nucleaz moisture-density tests (SLDb-139 and SLDb-144) failed to meet project specifications <br />with regard to compaction. The areas were reworked, retested, and met the requirements of the <br />• specifications.
