Phase IVb Weekly Summary No. 32 Page 3 -' 003-2203
Field Monitoring and TestingActivifies
Golder observed construction activities associated with Phase IV earthworks. All construction materials
and soil materials placed were verified as suitable, and construction methods were in accordance with
project specifications.
Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA
and the ADR Liner Expansion area for subgrade preparation, thickness, compaction, moisture
content, and materials. The Golder CQA Earthworks monitor performed thirty-two nuclear
moisture-density tests (SLNb-234 through SLNb-265) and performed eight nuclear moisture-density
retests on SLF that required reworking (SLNb-234a, 235a, 236a, 237a, 240a, 261 a, 262a, and 265a)
located within Phase IVb. The SLF met the requirements of the specifications prior to deployment
of geomembrane. Field test results are presented in Attachment B.
Golder observed placement of secondary and tertiary HVSCS piping in the Phase IVb OSA,
verifying construction and layout per the project plans.
Golder visually observed placement and grading of DCF in the Phase IVb OSA. Golder CQA
moiutors (one monitor per dozer when dozers are greater than 50 ft apart) observed DCF placement
and verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations,
5 feet for 773 equipment, and 10 feet for haul road construction and equipment.
Golder visually observed removal of DCF from the Phase IVb OSA tie-in areas and Phase I /ADR
Liner expansion tie-in areas. Golder marked damage caused to the geomembrane for subsequent
repair during ADR geomembrane tie-in construction.
Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension
(2' wide by 3' deep}. Anchor trench excavation met or exceeded the requirements of the
specifications.
Golder observed geomembrane deployment and repairs, documenting ambient air temperature, panel
dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and
destructive). Geomembrane placed in the Phase IVb OSA met project specifications. Golder also
observed the repair and testing of damaged SO-mil SST anchor trench shoulder near the LVSC riser
pipes.
Golder observed placement of SSF and geogdd for underground working remediaion. These
materials were placed in accordance with the drawings and specifications. Materials supplied met
specifications.
rnazc rvts2 sep¢mba~a-s~~m,eRn.zoos Golder Associates inc.
