June 24,2003 -6- 003-2203.7
• If construction and/or specification modifications were warranted due to site-specific conditions,
documentation was submitted to CC&V for DMG approval (see Appendix Q). No changes were
implemented without DMG approval. A summary of items that were addressed during construction
are summarized below as follows:
• April 2002: Golder accepted a variance in the Fine Shaft Backfill (FSB)
gradation for the specified quantity of material tested. The variance in the FSB
gradation was considered insignificant by Golder based on experience with
similar projects.
• August 2002: Golder accepted Leak Detection Fill (LDF) for the construction of
a section of the Leak Detection Trench (LDT) that was slightly more coarse than
the project specifications.
• October 2002: Ames installed a rock drain near the Phase IVb PSSA sump. The
trench and sump were constructed in accordance with the Certifying Engineer's
requirements.
• May 2003: Golder submitted a specification for Select Drain Cover Fill (SDCF)
for construction of the PSSA riser fill,
• May 2003: Golder approved removal of approximately 350 linear feet of 4-inch
• corrugated slotted HDPE Low Volume Solution Collection piping within the
southern portion of the PSSA sump area.
• June 2003: Golder approved substitution of 100-mil single-sided textured
geomembrane (textured side up) in place of 100-mil double-sided textured
geomembrane on a portion of the PSSA sump floor and in areas of double-sided
smooth .
• June 2003: Golder approved removal of the recessed depressions beneath the
PSSA riser base plates, whereby additional haunching of the High Volume
Solution Collection pipes is required.
• June 2003: Golder approved replacement of 26-inch HDPE pipe lengths entering
the Phase N PSSA sump with steel piping.
• June 2003: Golder approved the replacement of two 4-inch PCPE pipes in place
of each 6-inch PCPE pipe in the south west and south east risers.
1.5 Record Survey Documeutation
Ames Construction of Aurora, Colorado provided the Record Drawings for the activities associated
with the Phase IVb PSSA. A Colorado Registered Land Surveyor prepared the drawings. The Record
Drawings are presented in Appendix A. The Surveyor's certi£cates are presented in Appendix F,
Ames also provided locations for SLF density tests, SLF depth checks, geomembrane panels, and
I:\00@203\OJ00\FnICertP5SA0603\W3Ii03.M00A~3URep.J« Golder AssOCiOtes
