Appendix C presents a summary of the Golder staff on-site during <br />the period of August 30, 2002 through June 2003. <br />Representatives of CC&V, Ames, Poly-Flex, and Golder conducted daily, weekly, and <br />preconstruction coordination meetings throughout the construction period. <br />1.3.1 Daily Construction Summary Re orts <br />Golder prepared daily construction summary reports as well as photographic documentation <br />(Appendix B) throughout the project to document the key elements of construction and the CQA <br />activities that occurred. On-site Golder CQA personnel, the Certifying Engineer and the Project <br />Engineers discussed construction activities on a regular basis. In addition, field personnel prepared <br />daily observation reports. Golder submitted these daily reports and a daily summary report to CC&V. <br />• Daily reports are not included in this report but were available for Colorado Division of Minerals and <br />Geology (DMG) review during the course of the project. <br />1.3.2 Weekly Construction Summary Reports <br />Golder prepared weekly construction summary reports which were submitted to CC&V and to the <br />DMG. Weekly Reports from the period of February 2001 through August 29, 2002 are presented in <br />the 2002 Phase Na Certification Report. The weekly reports during the period of August 30, 2002 <br />through June 24, 2003 (including Phase Nb and c construction activities) are presented in <br />Appendix D. <br />1.4 Project Specifications <br />All work was performed in accordance with the design plans and specifications in Amendment No. 8 <br />to Mining Permit M-1980-244 and subsequent revisions as approved by DMG.
