Ames continued haul and placement of DCF <br />onto the north slope of the Phase IVb OSA, west of the LOB. Ames also continued haul and placement <br />of DCF in the Phase IVb OSA adjacent to the Phase IVa tie-in. <br />Ames continued production of DCF at the screen plant located near the Secondary Crusher. Ames also <br />continued production of SLF. <br />Ames placed and compacted Bedding Fill (BF) material into the Phase IVb OSA on the central azea of <br />the Phase III ore slope in prepazation for geomembrane deployment. <br />Poly-Flex continued deployment of 80-mil double-sided textured LLDPE geomembrane in the Phase <br />Nb OSA on the 2H:1 V riser slope. Poly-Flex deployed 30-miI double-sided textured LLDPE <br />geomembrane over an area of approximately 23;091 square feet in the Phase Nb OSA during this <br />reporting period. Geomembrane panels B-301 through B-308 were deployed, seamed, and tested by <br />Poly-Flex. Destructive fusion and extrusion seam samples DFb-98 through DFb-101, and DXb-20 <br />through DXb-21 were marked and removed. Destructive fusion and extrusion seam peel and sheaz tests <br />on 80-mil double-sided textured LLDPE geomembrane deployed during this reporting period passed <br />per project specifications. <br />A map of the area worked during this reporting period is provided in Attachment C. <br />Phase IVc <br />Ames performed no construction activities in the Phase IVc area during this reporting period. <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed placement of secondary and tertiary HVSCS piping in the Phase IVb OSA, <br />verifying construction per the project plans. <br />Golder visually observed placement of DCF in the Phase IVb OSA. Golder CQA monitors (one <br />monitor per dozer when dozers are greater than 50 ft apart) observed DCF placement and verified <br />minimum thickness of 2 feet for finish grade, 4 feet for HV"SCS primary pipe locations, 5 feet for <br />773 equipment, and 10 feet for haul road construction and equipment. <br />Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension <br />(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications.
