Corps of Engineers, Sacramento District <br />Western Colorado Regulatory Office <br />402 Rood. Avenue, Room 142 <br />Grand Junction, Colorado 81501-2563 <br />(303j 243-1199 <br />The Corps of Engineers will review your request and inform <br />you, in writing, that the work may proceed under the <br />authorization given in this permit. An anolicant may not start <br />work until notified by the Coros of Engineers. If the work is <br />not covered by this permit, you will need to apply for an <br />individual Department of the Army permit. <br /> <br />THIS REGIONAL GENERAL PERMIT IS SUBJECT TO THE FOLLOWING <br />CONDITIONS' <br />1. You must submit a written description of the proposed <br />activity to the Colorado Division of Wildlife and obtain a formal <br />certification or waiver of approval for the activity. This <br />should be accomplished well in advance of your application to the <br />Corps of Engineers in order to avoid a delay in the processing of <br />your permit request. The Colorado Department of Transportation <br />may fulfil this requirement in accordance with its programmatic <br />Senate Bill 40 i3emorandum of Agreement with the Colorado Division <br />of Wildlife. <br />2. The Colorado Department of Health has certified the <br />activities authorized by this permit under Section 401 of the <br />Clean Water Act. 7Phe Environmental Protection Agency must <br />individually certify the activities on Indian lands authorized by <br />this permit. To obtain water quality certification for your <br />activity on Indian lands, you must write to the State Program <br />Management Branch, Environmental Protection Agency, Region VIII, <br />999 Eighteenth Street, Suite 500, Denver, Colorado 80202-2466. . <br />Your certification from EPA on Indian lands should accompany your. <br />application for approval from the Corps of Engineers. <br />3. The permi.ttee shall exercise all necessary standard <br />construction procedures to protect waters of the United States <br />from pollution by contaminants and to minimize turbidity and <br />siltation of these waters. To the maximum extent practicable, <br />discharges of dredged and fill material into waters of the United <br />States shall be avoided or minimized through the use of <br />alternatives. <br /> <br />4 <br />
