c. Description of the proposed changes, including <br />the type and amount of wetlands mitigation, <br />and how these changes will be accomplished; <br />d. Description of the site preparation <br />requirements; <br />e. Description of your revegetation/planting <br />scheme; <br />f. Schedule for accomplishing the mitigation work <br />and your expectations for final success, <br />including functions and values (if known); <br />g. Description of any operation and maintenance <br />requirements for the mitigation site; <br />h. Monitoring plan for the mitigation including <br />measurable success criteria and any <br />contingencies if success criteria are not met; <br />i. Proposal for periodic reporting to the Corps of <br />Engineers during and after accomplishment of <br />the mitigation and; <br />j. A set of illustrative drawings of the location <br />and plans for mitigation. <br /> <br />All drawings should be on paper that is 8 1/2 inches by li inches <br />in size with all pertinent dimensions such as length, width and <br />depth of all fill work. A bar scale should be included on each <br />drawing. <br />The written notification requesting authorization under this <br />permit must be sent to the appropriate Corps of Engineers <br />District Office. Refer to the enclosed map showing the District <br />boundaries of the three offices operating in Colorado.
