6. Locate the disposal sites on a map and provide maps, cross sections, and descriptions of the final <br />configuration of disposal cells. The final graded surfaces of the debris must be free draining. <br />7. Provide a description of the cover soil that will cap the debris and a map and/or description of the <br />source azeas for the cover soil. <br />8. A cost associated with disposing the material on site will need to be included. The Division estimates <br />an on-site disposal cost for the 30,000 CY of concrete and asphalt is $23,000.00. A copy of <br />calculations used to determine this cost is attached for your review. <br />At this point in time the operator has two options, either submit an additional bond in the amount of <br />$342,000.00, or revise the reclamation plan to incorporate this material into the backfilling and grading (items <br />#1-8 above will need to be addressed). <br />Please be advised that Division's decision due date is set for June 11, 2003. If you feel more time is needed to <br />complete your reply, the Division can grant an extension to the decision date. This will be done upon receipt <br />of a written waiver of your right to a decision by June 11, 2003 and request for additional time. This must be <br />received no later than the deadline date.
