S 100 if converted during the first year of the permit ; <br /> $ 500 if converted during the second year of the permit : <br /> $1000 if converted during the third year of the permit ; <br /> $1500 1f converted during the fourth year of the permit : and <br /> S2000 if converted during the fifth year of the permit . <br /> 1 . 103 Contents of application <br /> (a) Except as otherwise indicated in this section, the operator <br /> shall provide all information required by the Act and these <br /> Rules for his size of operation. However, the operator <br /> need not supply an information required by the provisions of <br /> the 1976 .Act which he has previously supplied unless such <br /> information is different from that in the original application. <br /> (b) In addition , the application shall show: <br /> 1 ) the area mined or disturbed, and <br /> 2) the area reclaimed since the original permit application, <br /> 1 . 104 Applications for conversion will be reviewed by the Board as <br /> applications for new permits under Rules 2 and 3 except that con- <br /> versions of Rule 3, 1976 life of the nine permits , to Rule 2 permits <br /> must be acted upon within ninety (90) days of the filing thereof. <br /> - 12 - <br />
