S 100 if converted during the first year of the permit ;
$ 500 if converted during the second year of the permit :
$1000 if converted during the third year of the permit ;
$1500 1f converted during the fourth year of the permit : and
S2000 if converted during the fifth year of the permit .
1 . 103 Contents of application
(a) Except as otherwise indicated in this section, the operator
shall provide all information required by the Act and these
Rules for his size of operation. However, the operator
need not supply an information required by the provisions of
the 1976 .Act which he has previously supplied unless such
information is different from that in the original application.
(b) In addition , the application shall show:
1 ) the area mined or disturbed, and
2) the area reclaimed since the original permit application,
1 . 104 Applications for conversion will be reviewed by the Board as
applications for new permits under Rules 2 and 3 except that con-
versions of Rule 3, 1976 life of the nine permits , to Rule 2 permits
must be acted upon within ninety (90) days of the filing thereof.
