RULE 2 : PERMITS FOR REGULAAERATIONS • <br /> 112( I ) 2. 1 Application for a reqular permit will be required for any mining operation <br /> affecting ten acres or more , or extracting 70,000 tons or more of mineral , <br /> overburden , or combination thereof per calendar year. In this application , <br /> the operator shall submit four copies of the application form and attachments , <br /> as outlined in Section 2. 12 , to the offices of the Division and one additional <br /> copy to the appropriate county(s) , as set forth in Section 2.22. All tests , <br /> analyses , surveys and maps shall be prepared by qualified persons. All in- <br /> formation submitted in an application must he accurate and complete, and <br /> acknowledged as such by the sionature of an authorized individual . <br /> 2. 11 The application shall contain : <br /> 112( 1) ( 1 ) NAME OF OPERATION <br /> Mine name , site name , or other applicable designation for the <br /> area to be mined. <br /> (2) NAME OF APPLICANT <br /> The business organization or individual in whose name the <br /> permit will be issued. <br /> 112(2) (e) (3) ADDRESS AND PHONE NUMBER OF LOCAL OFFICES <br /> This pertains to the office closest to the proposed mining <br /> operation. <br /> 112(2) (e) (4) ADDRESS AND PHONE. NUMBER OF GENERAL OFFICES <br /> This pertains to the home office, if different from above. <br /> (5) NAME AND ADDRESS OF PERSON WHOM STAFF MEMRERS SHOULD CONTACT <br /> This is the person whom staff members should contact concerning <br /> this operation. <br /> (6) PARENT CORPORATION <br /> The name and address of parent corporation , if any. <br /> �112(2) (b) (7) OWHER(S) OF RECORD OF AFFECTED LAND (SURFACE AREA) <br /> If more than two. see EXHIP, IT P <br /> 112(2) (c) (8) 0�4NER(S) OF SUBSTANCE TO RE MINED_ <br /> If more than two , see EV IRIT PT <br /> 112 (2) (d) (9) SOURCE OF LEGAL RIGHT TO FNT_ER <br /> See EXHIBIT 0 <br /> (10) TYPE OF MINE AND SUBSTANCE TO RF. MINED <br /> Specify underground , strip , open pit , or other, and substance(s) <br /> to be mined. <br /> (II) GENERAL LOCATION AND ELEVATION OF OPERATION <br /> This should define the area in terms of miles and direction <br /> from nearest town and highway , and an approximate elevation. <br /> (12) COUNTY <br /> ( 13) ACREAGE OF AFFECTED LANDS <br /> The total surface acreage of lands affected by the operation. <br /> (See RULE I for definition. ) <br /> 112(6) ( 1.4) APPLICATION FEE <br /> The operator shall indicate the fee submitted with the appli - <br /> cation, computed as follows: 550 basic fee , plus 515 per acre <br /> for the first 50 acres , S10 per acre for the second 50 acres , <br /> $5 per acre for the third 50 acres , and SI per acre for any <br /> 4C <br /> ' 1S - <br />
