The exact due date for the <br />permittee's first annual report will be listed in their permit certification. The Division reserves the right to require <br />additional information in the report, on a case-by-case basis, as needed. <br />All reports required for submittal shall be signed and certified for accuracy by the permittee (see Pan I.D.9). <br />A signed copy of the above'report forms shall be submitted to the following address: <br />Colorado Department of Public Health and Environment <br />WQCD PERMITS <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />Test Procedures <br />Test procedures for the analysis of pollutants shall conform to regulations published pursuant to Section 304 (h) of the <br />Clean Water Act, and Colorado State Effluent Limitations Regulation 62, under which such procedures may be required. <br />6. Additional Monitoring by Permittee <br />If the permittee monitors any pollutant ar the location(s) designated herein more frequently than required by this permit, <br />using approved analytical methods as specified above, the results of such monitoring shalt be included in the calculation <br />and repotting of the values required in the Dischazge Monitoring Report Form or other forms as required by the <br />Division. Such increased frequency shall also be indicated. <br />Reoorting_to Municipality <br />Any permitted facility d'tschazging to a municipal storm sewer shall provide the municipality with a copy of the permi[ <br />application, upon request. A copy of the SWMP shall also be provided to the municipality upon request. <br />8. Sgecial Monitoring <br />a. TDS monitoring is only requred for facilities discharging in the Colorado River Basin. TDS may be measured as <br />electrical conductivity where a sa[isfactory correlation with TDS has been established based upon a minimum of <br />five (5) samples. See the individual Certification Rationale for a determination as to whether monitoring is <br />required. <br />b. Phosphorus monitoring is required only for facilities which dischazge into the following drainage basins: Chem <br />Creek basin, Chatfield Reservoir downstream of the USGS gage at Wa[erton and on Plum Creek, Dillon Reservoir <br />basin (i.e., Ten Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek <br />basin. See the individual Certification Rationale for a determination as to whether monitoring is required. <br />• <br />
