Failure to submit the reports az required in either Part I.D.3 or I.D.4 shall constitute a violation of the permit and may constitute grounds for revocation of the permittee's authority to discharge under the permit. 2. Sampling-Stotmwater Sampling and tes[ing of stormwater for specific parameters under Part I.C is not required on a routine basis under this permit. However, the Division reserves the right to require sampling azid~esting beyond [his, on a case-by-case basis, at any facility covered by this permit, in the event that there is reason to suspec[ that compliance with the SWMP is a problem, or to measure the effectiveness of the BMPs in removing pollutants in the effluent. 3. Quarterly Report -Process Water For facilities allowed [o discharge process water, reporting of the data gathered in compliance with Pan I.B.3.a shall be on a q»arterly basis. Monitoring results obtained during the previotu 3 months shall be s+++++++a*+~~d and repotted on Division approved Discharge Monitoring Report forms (DMRs), postmarked no later than the 28th day of the month following the completed calendar quaver. For example, far the 1st quaver (January, February, and March) the gttanerly report must be received at this office postmarked no later than Apri128. If no discharge occurs dtuing the reporting period, 'No Discharge" shall be repotted. 4. Re rtine of Data - Starmwater Duchareers a. Annual Report - SWMP The permittee will be required to submit an Annual Report, covering January 1 through December 31 of each year, on the overall compliance with the SWMP. The annual report will contain, at a minimum: 1) Name of permittee, address, phone number, and permit certification number. • 2) A report on the facility's overall compliance with the SWMP. 3) A summary of each comprehensive stormwater facility inspection made, including date, findings, and action taken.
