All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with <br />the following criteria: <br />1) In the case of corporations, by a principal executive officer of a[ least the level of vice-president or his or <br />her duly authorized representative, if such representative is responsible for the overall operation of the <br />facility from which the discharge described in the form origina[es; <br />2) In the case of a parmership, by a general parmer; <br />3) In [he case of a sole proprietorship, by the propriemr; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking <br />elected official, or other duly authorized employee. <br />b. Changes to authorvation. If an authorization under pazagraph a) of this section is no longer accurate because a <br />different individual or position has responsibility for the overall operation of the facility, a new authorization <br />satisfying the requirements of pazagraph a) of this section must be submitted to the Division, prior to or together <br />with any reports, information, or applications to be signed by an authorized representative. <br />c. Certification. Any person signing a document under pazagraph a) of this section (except for the SWMP <br />certification) shall make the following certification: <br />"k certify under penalty of law that this docttmen[ and all attachments were prepazed under my direction or <br />supervision in accordance with a system designed m assure that qualified personnel properly gather and <br />evaluate the information submitted. Based on my inquiry of the person or persotts who manage the system, <br />or those persons directly responsible for gathering the information, the information submitted is, to the best <br />• of my knowledge and belief, true, accurate, and complete. I am awaze that there are significant penalties <br />for submitting false information, including the possibility of fine and imprisonmen[ for knowing <br />violations." <br />E. DEFINITIONS <br />1. A "continuous" measurement, for flow monitoring requirements, is a measurement obtained from an automatic <br />recording device which continually measures flow. <br />2. A "grab" sample, for monitoring requirements, is a single "dip and take" sample. <br />3. An "in-situ" measurement, for monitoring requirements, is defined as a single reading, observation or measurement <br />taken in the field at the point of discharge. <br />4. An "instantaneous" measuremem, for monitoring requirements, is a single reading, observation, or meazuremem <br />performed on site. <br />5. When the measurement frequency indicated is quarterly, the samples shall be collected during Mazch, June, September <br />and December, if a continual discharge occurs. If the dischazge is in[ermittent, then samples shall be collected during the <br />period that dischazge occurs. <br />6. Salinity is measured as Total Dissolved Solids (TDS). <br />7. The seven ('n day average shag be determined by the arithmetic mean of all samples taken in a seven (7) day period. <br />. Samples may not be used for more than one (1) reporting period. <br />8. The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30) <br />consecutive-day period. Samples shall ao[ be used for more than oae (1) reporting period. <br />
