• 9. Sitmatorv Requirements
a. All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with
the following criteria:
1) In the case of corporations, by a principal executive officer of a[ least the level of vice-president or his or
her duly authorized representative, if such representative is responsible for the overall operation of the
facility from which the discharge described in the form origina[es;
2) In the case of a parmership, by a general parmer;
3) In [he case of a sole proprietorship, by the propriemr;
4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking
elected official, or other duly authorized employee.
b. Changes to authorvation. If an authorization under pazagraph a) of this section is no longer accurate because a
different individual or position has responsibility for the overall operation of the facility, a new authorization
satisfying the requirements of pazagraph a) of this section must be submitted to the Division, prior to or together
with any reports, information, or applications to be signed by an authorized representative.
c. Certification. Any person signing a document under pazagraph a) of this section (except for the SWMP
certification) shall make the following certification:
"k certify under penalty of law that this docttmen[ and all attachments were prepazed under my direction or
supervision in accordance with a system designed m assure that qualified personnel properly gather and
evaluate the information submitted. Based on my inquiry of the person or persotts who manage the system,
or those persons directly responsible for gathering the information, the information submitted is, to the best
• of my knowledge and belief, true, accurate, and complete. I am awaze that there are significant penalties
for submitting false information, including the possibility of fine and imprisonmen[ for knowing
violations."
E. DEFINITIONS
1. A "continuous" measurement, for flow monitoring requirements, is a measurement obtained from an automatic
recording device which continually measures flow.
2. A "grab" sample, for monitoring requirements, is a single "dip and take" sample.
3. An "in-situ" measurement, for monitoring requirements, is defined as a single reading, observation or measurement
taken in the field at the point of discharge.
4. An "instantaneous" measuremem, for monitoring requirements, is a single reading, observation, or meazuremem
performed on site.
5. When the measurement frequency indicated is quarterly, the samples shall be collected during Mazch, June, September
and December, if a continual discharge occurs. If the dischazge is in[ermittent, then samples shall be collected during the
period that dischazge occurs.
6. Salinity is measured as Total Dissolved Solids (TDS).
7. The seven ('n day average shag be determined by the arithmetic mean of all samples taken in a seven (7) day period.
. Samples may not be used for more than one (1) reporting period.
8. The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30)
consecutive-day period. Samples shall ao[ be used for more than oae (1) reporting period.
