M. S. Hayes <br />Page 3 <br />June 29, 1990 <br />14. See revised page 4.05-10. <br />15. Page 25 of Exhibit 14 does not specificall,v state that <br />Spring G-26 will be impacted. This page discusses the location <br />of the spring relative to the underground mine and addresses <br />changes in the mine plan that will minimize the possibility that <br />the spring will be impacted. Section 5, Page 4 in Exhibit 14 has <br />been revised to reference Spring G-26 which is within the subsi- <br />dence boundary. The Division's discussion in this paragraph <br />implies that there are additional springs within the subsidence <br />boundary. If this is true, please inform the Operator of their <br />location so they can be evaluated. The Operator is not aware of <br />any springs within the subsidence boundary other than Spring G- <br />26. <br />16. Page 2.04-32 was revised to reference the location of <br />the sediment and drainage control plan. A new sediment and <br />drainage control plan is enclosed as Exhibit 11. <br />17. Page 2.05-22R and 4.04-5 now reference Exhibit 11 as <br />the location of the new sediment and drainage control plan. <br />18. The commitment to monitor the sediment pond periodical- <br />ly contained on page 2.05-22R was revised to quarterly. <br />19. The schedule presented on pages 2.05-22R and 4.05-6R <br />for sediment removal was revised from annual to periodic. Page <br />4.05-6R was revised to indicate that the sediment removed from <br />the pond will be disposed of at the Somerset Mine, Elk Creek <br />Facilities, gob disposal area. <br />20. On May ]4, 1990, the Operator submitted an application <br />for reissuance of it NPDES permit. A cop3' of the reissued permit <br />will be submitted to the Division as soon as it is received from <br />the Colorado Department of Health. <br />21. A new sedimentation control system map and plan is <br />included as Exhibit 11. Pages 2.05-55, 4.05-3 and 4.05-5 were <br />revised to include the location of the new plan. <br />22. A map and narrative of the Access Road Small Area <br />Exemption is included as Exhibit 11. A discussion of the small <br />area exemption was added to page 4.05-5. <br />23. A commitment to fill and regrade gullies deeper that 9" <br />was added to page 4.14-2. <br />24. Ute Engineering and Surveying performs subsidence <br />surveys aver Section 17 twice a ,year. Discussions with their <br />surveying staff indicate that there are not any stock ponds <br />overlying the mine in Section 17. <br />
