Page Five <br />2/10/92 Mr. Steen said Mr. Hendricks told him that COM, Inc. took 22 loads, <br />(8 wheel dump truck) of ore to mill - actually 400 tons ore, 150 <br />tons waste - and are processing it at the mill. They picked up the <br />waste by mistake. <br />2/11/92 Mr. Hendricks told the Division, COM, Inc. picked up 436 tons of <br />ore and waste on 1/17/92 and 1/18/92 - used Keeter Trucking. <br />(442-2131) <br />2/13/92 Mr. Tatman told Division he stopped processing Bueno ore on date of <br />inspection. <br />2/13/92 Division follow-up inspection - All six possible problems and <br />violations not adequately addressed. The ownership of ore <br />processed at the mill not addressed by COM, Inc., in writing. <br />Two additional concerns identified on the inspection: <br />(1) PB - Upland Diversion Ditch around tailings impoundment <br />plugged. Clean and maintain by 3/24/92. <br />(2) PV - Uncontrolled tailings spill on ground next to mill and <br />tailings impoundment. Submit a detailed plan for clean-up of <br />spills and a detailed plan for future containment of tailings - <br />including back-up measures to be taken in line breaks, tank <br />overflows and other spills. Submit a map showing all water <br />monitoring wells and recent water monitoring data collected at each <br />well. <br />2/19/92 Possible violation - Continued to March Board Meeting to <br />investigate 2/13/92 inspection issues. <br />2/27/92 Rob Helmick, Boulder County Land Use Dept. said Mr. Tatman and Mrs. <br />Fraser have not adequately addressed county concerns. <br />Mr. Tatman said he was pumping from the Cash Mine to the Mill for <br />process water.
