• 4.4 Phelps Dodge NotiScations and Internal Contacts: <br />Phelps Dodge Corporate Release Reporting Procedures (Appendix 1) will be followed in the <br />event of a petroleum or chemical spill. No agency notification will be performed prior to <br />completing a "Spill Information Form", which is provided in Appendix 1. The completed form <br />provides information that must be verbally reported to an agency if the spill is reportable. These <br />reports must be made as soon as possible, but at a time when doing so does not interfere with <br />spill response activities. The federal threshold for verbally reporting a petroleum spill is tied to <br />quantities that aze "harmful". Harmful quantities aze defined by spills that: <br />1. Cause a violation of applicable water quality standards, <br />2. Cause a film or sheen on waters of the U.S., or <br />3. May affect natural resources of the U.S. <br />A trained member of the emergency response team, in coordination with the Environmental <br />Manager, will normally make this determination if the Site Manager is unavailable. In all cases it <br />is imperative that the Environmental Manager be included in the decision making process. <br />Verbal notifications are required according to the procedures described below. Notification to <br />the agencies for various spill conditions is required only if an emergency condition exists which <br />the Site Manager and/or the Environmental Manager believe requires assistance from outside <br />agencies. Agencies may be contacted otherwise if it is determined that a courtesy call to the <br />• agency is appropriate. <br />If written follow-up to the spill is requested by an agency following the verbal notification, a <br />copy of the Spill Information Form (Appendix 1) may be used It is expected that such a <br />request will only be made by the Colorado Department of Health and Environment under the <br />hazardous materials spill reporting requirements.
