4.5 Reporting Large and Frequent Spills <br />If a spill of more than 1000 gallons of oil products occurs in any event, or if there aze two spill <br />events in a twelve month period, each with greater than 42 gallons spilled, then 40 CFR Part <br />§ 112.4 requires submitting to the Regional Administrator of EPA the following information: <br />1. Name of facility; <br />2. Your name; <br />3. Location of facility; <br />4. Maximum oil storage or handling capacity of facility; <br />5. Corrective action and countermeasures you have taken, including a description of equipment <br />repairs and replacements; <br />6. A description of the facility, including maps, flow diagrams, and topographical maps; <br />7. The cause of such discharges, including a failure analysis of the system in which the failure <br />occurred; <br />8. Additional preventive measures taken or contemplated to minimize the possibility of <br />recurrence, and <br />9. Such other information as the Regional Administrator may reasonably require pertinent to the <br />plan or dischazge. <br />4.6 Spill Control Services and Material Suppliers <br />Climax personnel and equipment should always be used first in any spill clean-up effort. Spill <br />kits aze located conveniently around the site (see Sections 5 and 6). In the event that special <br />equipment or materials aze needed, the following companies may be contacted.
