All reports required (or submittal shall be signed and cemfi~xl for accuracy by the permittee in accordance with <br />the following criteria: <br />1) In the case of corporations, by a principal executive officer of et least the level of vice-president or his or <br />her duly authorized represwtative, if such representative is responsible (or the overall operation of the <br />facility from which the discharge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor, <br />4) In the case o(a municipal, state, or other public facility, by either a principal executive officer, ranking <br />elected official, or other duly authorized employee. <br />i <br />b. Chtrnges to authorization. if en authorization under paragraph e) of this section is no longer accurate because <br />a different individual or position has responsibility for the overall operation of the facility, a new authorvatioo <br />satisfying the requiretnertts of paragraph a) of this section must be submitted to the Division, prior to or together <br />with any reports, information, or applications to be signed by an authorized representative. <br />e. Ctrtifitalion. Any person signing ^ d«umeitt under paragraph a) of this section (except for the SWMP <br />certification) shall make the following certification: <br />'1 certify under penalty of law that this d«urix;nt and all atlachmenls were preparexl under my direction <br />or supervision in accordance with a system designed to assure that qualified personnel properly gather <br />and evaluate the information submitted. Baseid on my inquiry of the person or persons who manage the <br />system, or those persons directly responsible for gathering, the information submitted is, to the best of <br />my knowledge and belief, true, accurate, andcomplete. I am aware that there ere significant penalties <br />for submitting false information, including the possibility of fine and imprisonment for knowing <br />violations.' <br />E. FOOTNOTES -APPLICABLE TO PRECEDING PAGES <br />a/ The chitty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30) <br />consecutive-day period. Samples shall not be used far more than one (1) reporting period. <br />b/ The seven (7) day average shall be determined by the arithmetic mean of all samples taken in a seven (7) day period. <br />Samples rosy not be used (or more than one (1) repirting period. <br />c/ This limitation shall be determined by a single sample or set of samples as required by Part I.B.3, Sample Type. <br />I <br />d/ Monitoring is required otily during periods of discharge. If no discharge occurs, this shall be reported at the specified <br />frequency. (See Part LD.3.) ~ <br />I <br />e/ TDS monitoring is only required for facilities discha grog io the Colorado River Basin. TDS tray be measured as <br />electrical conductivity where a satisfactory correlation with TDS has been established based upon a minimum of five <br />(5) samples.
