The Division reserves the right to roquire additional information in the report, on a <br />case-by~ase basis, as needed. <br />All reports required for submittal shall be signed and certified for accuracy by the permittee (see Part I.D.LO). <br />A signed copy of the above report forms shall be submitted to the following address: <br />Colorado Department of Health <br />Water Quality Control Division <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Deaver, Colorado 80222-1530 <br />Attention: Permits and Enforcement Section <br />Test Procedures <br />Test procedures (or the analysis of pollutants shall conform to regulations published pursuant to Section 304 (h) of the <br />Clean Water Act, and Colorado Stale Effluent Limitations (10.1.5), under which such procedures may be required. <br />Recordine of Results <br />For each measurement or sample taken pursuant to the requirements of this permit, the permillee shall record the <br />following information: <br />a. The enact place, dale, end time of sampling; <br />b. The dates the analyses were performed; <br />c. The person(s) who performed the sampling; <br />d. The person(s) who performed the analyses; <br />e. The analytical techniques or methods rased; and <br />f. The results of all required analyses. <br />Additional Dtonilorintt by Permitlce <br />If the permillee nronilors any pollutant at the location(s) designated herein more frequently t}tan required by this permit, <br />using approved analytical methods as specified above, the results of such monitoring shell be included in the calculation <br />and reporting of the values required in the Discharge Monitoring Report Form or other forms es required by the <br />Division. Such increased frequency shall also be indicated. <br />g. Records Retention <br />All records end information resulting from the monitoring activities required by this permit including all records of <br />analyses performed end calibration and maintenance of instrumentation and recordings from continuous monitoring <br />instrumentalioo shall be retained (or a minimum of throe (3) years, or longer if requested by the Regional <br />Administrator or dte State Water Quality Control Division. <br />9. Repotting to Atunicioalilr <br />Any permitted facility discharging to a municipal storm sewer shall provide the municipality with "a copy of the permit <br />application, upon request. A copy of the SWMP shell also lx provided to the municipality upon request. <br />
