(See Part 1.D.10 of the permit.) a. Quarterly Report -Process Water For (ecilities allowed to discharge process water, reporting of the data gathered in compliance with Part I.B.3.a shell be on a quarterly basis. 4. Monitoring results obtained during the previous 3 months shall be summarved end reported oo Division approved Discharge Monitoring Report forms (DMRs), postrarked mo later than the 28Ut day of the month following the completed calendar quarter. For example, for the 1st quarter (January, Febrtatry, and March) the quarterly report must be received et this office postmarked no later than April 28. If no discharge occurs during the reporting period, 'No Discharge' shall be reported. b. Annual Report -Process Weler For facilities not allowed to discharge process water (see Part ].B.3.6 of the permit), a report on whether or not a discharge of process water has taken place at the facility is r~uired, in letter form, on an annual basis. The report shall cover October 1 through Septembers 30 of each year, and will be due to the Division by the following October 28, beginning with the first year or partial year of permit coverage. The Division reserves the right the require additional information in thie report, on a case-by-case basis, as needed. Reporting of Data - Slormwater Discltareers a. Annual Report - SWAiP The permiltee will be required to submit an annual report, covering January 1 through December 31 of each year, on the overall compliance with the SWh1P. The annual report will contain, at a minimum: 1) Neme of permittee, address, phone number, and permit certification number. i 2) A report on the facility's overall complialnce with the SWMP. 3) A summery of each comprehensive storiwater facility inspection mazle, including dale, findings, and action taken. 4) Results end interpretation of any stormwater nanitoring performed. i The annual report will be due to the Division on or be (ore February 15 of each year, alter the first full year of coverage under the permit.
