COLORADO DEPARTMENT OF HEALTH - Watn• Quality Control Division <br />Rationale -Page 6 Permit No. COR-040000 <br />Division. (Note: Under the Federal regulations, this is referred to as a Norio of Intent, or NOL For internal <br />consistenry with its curreru program, the Division will cominue to use the term 'application.') This application will be <br />evaluated utilizing the criteria orulined perviously. <br />A. Aonlication <br />711e application shall require, at a minimum; <br />1. Name, and address ojthe applirnnt; <br />2. Site address and location; <br />3. Facility comas person and telephone numlxr; <br />4. Standard Irrdustrtal Class~exuion (S/C) code(s); <br />S. TIu size of the mining operation !n acxs; <br />6. Status as active or inaaivr; <br />7. List of any other environmental permits currently held by the facility; <br />8. Activities which rake glare at the site; <br />9. Receiving waters; and <br />10. For new active mining operations (beginning operations offer ]0/1/92) or uisring facilities applying after <br />7/1/93, ocrt frcntion as to the preparation oja SWMP. (See Seaton V/LB of the Rationale for SWMP <br />ratification requiranems for aisting mining operations applying on tune). Inaelivt mining operations <br />applying afro July 1, 1994 must submit a rertifrcntion as to the preparation and implementation of a SWMP. <br />B. Proeessinv Time <br />If this general permit is suitable far then applicant's operation, then a cert frcation will be developed and the <br />applicant will be crrtfred under this general pnmil. <br />the Division shall have up to thirty days «fler rcreipt of the above inforration ro request additional data and/or <br />deny the authorization for any particular disdtarge. Upon rcreipt of additional information, the Division shall <br />have an additional fifteen days to issue or deny authoriuuion far the particular discharge. <br />/f the applicant does not rereive a request for additional information or a nothcation of denial from the bivision <br />dated within thirty days ojthe rcreipt njthe application by the Division, authorization to discharge in accordance <br />with the conditions of this pnmit shall be deaned grained. <br />If, after evaluation of the application (or additional information, such as the SWMPJ, it is found that this genera[ <br />permit is not suitable jot the operation, then the application may be processed err one for an individual permit, or <br />amore appropriate general permit. Tke applicant will be noted of the Division's decision to deny cent frcation <br />under this general permit. For an individual permit, additional information may be requested, and 180 days will <br />be required to process the application and 'issue the permit. Temporary Coverage under the general permit will be <br />allowed umil the individual permit goes imo e~2a. (See discussion, section ILA of the Rationak.J <br />C. Chance in Facility Conditions or Operations <br />While tree fred under this general permit, if the mining opnation changes such that coverage under this general <br />permit is no bnger applicnbk, the permittee will be required by the Division to apply for an individual permit, or <br />seek coverage under a different general permit. Coverage will cominue under this general permit umi! issuance of <br />the other permit or rat cation. <br />VII. STORMWATER MANAGEMENT PLANS <br />A stormwater managemem pbn (SWMP) shall be developed and implemented for each mine covered by this permit.
