an injection well within 800 feet of the Dunn well. <br />Plans for cementing the Dunn well shall be sent to EPA <br />for approval prior the cementing operation. No other <br />corrective action is required. <br />C <br />WELL OPERATION <br />1. Prior to Commencino Infection. Injection operations <br />shall not commence into any newly drilled or converted <br />well until the operator has completed the following: <br />(a) Formation data, corrective action, and injection <br />fluid analysis have been submitted or complied <br />with in accordance with Part II Sections A.5., <br />B., and C.6., respectively; <br />(b) All pertinent logs and test(s) run during the <br />drilling and completion of the well, including <br />descriptive reports as required in Part II A.6., <br />have been submitted as well as the daily drilling <br />and cementing records. <br />(c) Mechanical integrity of the well has been <br />successfully demonstrated pursuant to Part II <br />C.2. and in accordance with the Guidance found in <br />Appendix F and evidence thereof submitted. The <br />operator shall notify the EPA thirteen (13) days <br />prior to conducting the test(s) so that an <br />authorized representative may be present to <br />witness the test(s); and, <br />(d) Construction is complete and schematic (wellbore <br />diagram after construction) has been submitted to <br />the Director with a notice of completion of <br />construction using EPA Form 7520-10 (see Appendix <br />B) and either: <br />(i) The Director has inspected or otherwise <br />reviewed the new injection well and finds it <br />is in compliance with the conditions of the <br />permit and the operator has received written <br />authorization to commence injection; or, <br />(ii) The operator has not received, within <br />thirteen (13) days of the date of the <br />Director's receipt of the report required <br />above, notice from the Director of his or <br />her intent to inspect or otherwise review <br />the new solution mining well(s), in which <br />Page 11 of 120 <br />EPA FINAL Area Permit No. C03870-00000 <br />
