case prior inspection or review is waived <br />and the operator may commence injection. <br />2. Mechanical Integrity. The operator shall establish <br />and shall mairifairi mechanical integrity of all wells <br />covered under this area permit in accordance with 40 <br />CFR Part 146.8. The mechanical integrity (MI) <br />demonstration consists of two parts: <br />(a) Part I MI demonstrates no significant leaks in <br />the casing, or tubing. The operator shall <br />demonstrate Part I MI by using the standard <br />annulus pressure test with a temporary packer or <br />other test approved by the Director. <br />(b) Part II MI demonstrates no significant fluid <br />movement into an underground source of drinking <br />water (USDW) through vertical channels adjacent <br />to the wellbore. The operator shall demonstrate <br />Part II MI by running a temperature (or other <br />approved) log survey after 60 days but no later <br />than six (6) months after injection commences. <br />If construction of the casing and tubing preclude <br />the use of a temperature log, a noise or an <br />oxygen activation log together with cementinc <br />records and the appropriate cement bond or cement <br />evaluation log will be utilized for making the <br />demonstration. <br />(c) The operator is required to pass both parts of <br />the mechanical integrity demonstration in <br />accordance with 2. (a) above prior to commencing <br />injection and 2.(b) above after the stated period <br />of operation. Thereafter the Part I MI pressure <br />test utilizing a temporary packer or an approved <br />alternative, and a temperature survey or other <br />approved Part II MI test, must be passed once <br />every five (5) years from the date of the last <br />approved demonstration. Test results shall be <br />submitted (on EPA form found in Appendix B) to <br />the Director as soon as possible but no later <br />than sixty (60) days after the test is complete. <br />A mechanical integrity test may be required after <br />workovers. However, removal or'in-place repair <br />of tubing that does not affect the isolation of <br />the well from aquifers will not require a <br />mechanical integrity test after workover of the <br />tubing. <br />Page 12 of 120 <br />EPA FINAL Area Permit No. CO3870-00000 <br />
