sampling is performed, this information must be <br />submitted as part of the well completion records. <br />6. Loggino Specifications and Coring Programs. <br />Appropriate open- and cased-hole geophysical well logs <br />and other tests shall be run. This well log suite <br />shall be tailored to provide the data necessary to <br />ensure adequate protection for all USDWS that will be <br />penetrated by solution mining well(s). A descriptive <br />report interpreting results of logs and tests shall be <br />prepared by a knowledgeable log analyst and submitted <br />to the Director. At a minimum, the following logs and <br />tests shall be conducted: <br />• Gamma Ray (GR) ; <br />• A cement bond log (CBL), conducted according <br />to EPA Guidance found in Appendix D, and/or <br />a cement evaluation test (CET) on the long <br />string casing(s) to ensure aquifer <br />protection; and <br />• Following two (2) but no later than six (6) <br />months after initial injection, run a <br />Temperature Log to ensure that there is no <br />upward movement of fluid behind the casing; <br />see Appendix E for Guidance. <br />Cement bond (and/or cement evaluation) and temperature <br />logging plans must be submitted to the EPA for <br />approval at least 13 days prior to logging. Tf the <br />construction of the well precludes the use of a <br />temperature log, a noise log or an oxygen activation <br />log together with cementing records and the <br />appropriate cement bond/evaluation log will be used to <br />demonstrate no fluid flow adjacent to the casing. <br />Coring may be performed in the mining interval as <br />needed to obtain pertinent stratigraphic data for <br />mining planning. <br />B. CORRECTIVE ACTION <br />The operator shall employ due diligence to verify that <br />the John E. Dunn Core Hole #20-1 is cemented below the <br />Dissolution Surface and provide the results (records <br />and information) to the Director. If not cemented in <br />a manner that prevents fluid migration out of the <br />mining zone, the John E. Dunn Core Hole #20-1 shall be <br />cemented throughout the Saline zone prior to operating <br />Page 10 of 120 <br />EPA FINAL Area Permit No. C03870-00000 <br />
