• • PART I <br />Page 5 <br />Permit No. COG-500000 <br />A. COVERAGE UNDER THIS PERMIT <br />the particular discharge. (Notification of denial shall be by letter, in cases where coverage under an eltemate <br />general permit or an individual permit is required, instead of coverage under this geoerel permit) <br />b. Automatic Coverage: If the applicant does not receive a request for additional information or a notification <br />of denial from the Division dated within thirty days of the receipt of the application by the Division, <br />authorization to discharge in accordance with the conditions of this permit shall be deemed I;ranted. <br />c. Individual Permit Required: If, after evaluation of the application (or additioml information, such as the <br />SWMP), it is found that this general permit is not applicable to the operation, then the application will be <br />processed as one for an individual permit. The applicant will be notified of the Division's decision to deny <br />certification under this general permit. For en individual permit, additional information may: be requested, and <br />180 days will be required to process the application and issue the permit. Temporary covertge under this <br />general permit will be allowed until the individual permit goes into effect. <br />d. General vs. Individual Permit Coverage: Auy mover or operator authorized by this permit may request to <br />be excluded from the coverage of this permit by applying for an individual CDPS permit. 'fie owner or <br />operator shall submit m individual application w the Division, with reasons supporting the request. <br />e. Permit Cerliticatiorxs: The individual certification prepared for each applicant will indicate: whether Part I.B <br />of the permit which must be followed by that applicant. Part I.C must be followed by all p:rmittees unless <br />exempted as outlined in Section III.C of the Rationale. (In the absence of such indication firm the Division, <br />the pertaittee will be responsible for dernm•-"g and then complying with the applicable sections in Part I.) <br />in addition, all certnittees will be required to comply with Part I, subparts A, D, E and F, and all of <br />Part II of the permit. <br />6. Permit Expiration Date <br />Authorization to discharge under this general permit shall commence on October 1, 1992, and shall expire on <br />September 30, 1997. The Division must evaluate and reissue this general permit once every five years, and must <br />also recertify the applicant's authority to discharge under the geperal permit at such time. Therefore, any pertnittee <br />desiring continued coverage under the general permit must reapply by March 31, 1997. The Division will determine <br />if the applicant may continue to operate under the terms of the general permit. <br />An individtul permit will be required for any facility not reauthorized to discharge under the reissued general permit. <br />For facilities wishing to terminate authorization under the new permit, provisions of Part II.B.S.d •,vill be applicable. <br />7. Inactivation of Certification <br />The permittee may request inactivation of their certification under this permit, once the site has be.n fully released <br />from their reclamation bond requirements by the Colorado Division of Mining end Geology (CDN:G). See Part <br />II.B.S.d of the permit. <br />EFFLUENT LIMITATIONS AND CONDTI'IONS -PROCESS WATER DISCHARGE <br />('I'bis section (I.B) only applies to facilities with a discharge of process water.) <br />General Limitations <br />a. There shell be po discharge of sanitary waste waters from toilets or related facilities. <br />b. There shall be no discharge of floating solids or visible foam in other than trace amounts. <br />
