• ~ge 3 <br />Permit No. COG-500000 <br />PART I <br />A. COVERAGE UNDER THIS PERMIT <br />1. Industries Covered <br />Uuder tiffs general permit, the following types of facilities may be granted audroriution to discharge treated process- <br />generated wastewaters end/or stormweter runoff into waters of the state of Colorado: <br />Facilities engaged in the mining and processing of sand and gravel (including rock or stone used as aggregate), and <br />mining end quarrying of odter nonmetallic minerals (except fuels or graphite}, as described by the Standard Industrial <br />Classification (SIC) Code 14, and asphalt and concrete batch plants located at facilities covered under SIC code 14. <br />a. Stormwafer vs. Process water: When stormwater mixes with process water. the process ~+vater limitations <br />(Rut I.B of Ilte perrrtit) apply to the disrJtarge of that mixed water. Process waters from these facilities are <br />subject to effluent limitations under 40 CFR 436. <br />b. Process Water: For purposes of this permit, process generated wastewaters include: <br />1) Product wash waters; <br />2) Maintemnce/equipment wash waters; <br />3) Transport waters; <br />4) Scrubber waters (crushers or classifiers); <br />5) Mine dewatering (groundwater and/or nmoff); <br />6) Other process water as determined by the permit issuing authority; <br />7) Stormwater nmoff from mine or processing areas; and <br />g) Stormwater runoff which mixes with process gwersted wastewater prior to semplin;;. <br />c. Stormwatrr: The stormwater section (Part I.C of the permit) is intended to cover stormwater runoff from <br />those portions of a nonmetallic minerals production operation that are not subject to effluent limitations under <br />40 CFR 436. <br />2. Dredce and Fill Ooetations <br />This permit does not constitute authorization under 33 U.S.C. 1344 (Section 404 of the Clean Wraer Act) of any <br />stream dredging or filling operations. <br />3. Anolication Reauiranents <br />In order to be considered eligible for authorization to discharge coder the terms and conditions of this permit, the <br />owner, operator, and/or authorized agent of any facility desiring to discharge must submit, by letter or hand delivery, <br />the discharge application form, available from the Water Quality Control Division (the 'Division'), which generally <br />requires the following information: <br />a. Name, address, end descriptive location of the facility; <br />b. Name of principal in charge of operation of the facility; <br />
