reRMrr, PART I <br />Page Ic of Ig <br />hcfmlll No. CO-003?3(iH <br />Upon approval of the implementation plan by the Division, all ternts and conditions of the implementation plan, <br />including but not limited to the compliance schedule, will automatically become conditions of this permit. <br />No later than 14 calendar days following each date identified in the above schedule of compliance, the permittee <br />shall submit either a report of progress or, in the case of specific actions being required by identified dates, a <br />written notice of compliance or noncompliance, any remedial actions taken, and the probability of meeting the nett <br />scheduled requirement. <br />A schedule for the elimination of the dischazge, through connection to another treatment system or by other <br />means, may be substituted for this schedule of upgrading. <br />2. Reporting of Data <br />Reporting of the data gathered in compliaace with Part I.B.1 shall be on a monthly basis. <br />Monitoring results shall be summar'vod foreach calendaz quarter and reported on Division approved dischazge <br />monitoring report (DMR) forms postmarked no later than the 28t1t day of the following month following the end of <br />the quarter (for example, the DMR for the first caleodaz quarter must be postmarked by April 28th). Chronic WET Testime-Qtttfall(s): <br />(a) Testine and ReooRine Requirements <br />Test results shall be reported along with the Discharge Monitoring Repoli (DMR) submitted for the end of the <br />reporting period when the sample was taken. (i.e., WET testing results for the calendar quarter ending March <br />31 shall be reported with the DMR due April 28.) The results shall be submitted on the Chronic Toxicity Test <br />report form, available from the Division. Copies ofahese reports are to be submitted to both the Division amd <br />EPA along with the DMR. <br />
