General vs. Individual Permit Coverage: Any owner or operator authorized by this permit may request to be excluded from the coverege of this permit by applying for an individual CDPS permit. The owner or operator shall submit an individual application to the Division, with reasons supporting the request. e. Permit Certifira[ions: The individual cer[ifiration prepared for each applicant will indicate whether Part [.E of the permit which must be followed by that applicant. Part I.C must be followed by all permittees unless exempted as outlined in Section III.C of the Rationale. (In the absence of such indication from the Division, the permittee will be responsible for determining and then complying with the applicable sections in Part I.) In addition, all permittees will be required to comply with Part I, subparts A, D, E and F, and all of Part II of the permit. 6. Permit Expiration Date Authorization to discharge under this general permit shall commence on October 1, 1992, and shall expire on September 30, 1997. The Division must evaluate and reissue this general permit once every five years, and must also recertify the applicant's authority to discharge under the general permit at such time. Therefore, any pertnittee desiring continued coverage under the general permit must reapply by March 31, 1997. The Division will determine if the applicant may continue [o operate under the terms of the general permit. An individual permit will be required for any facility not reauthorized to discharge under the reissued general permit. For facilities wishing to terminate authorization under the new permit, provisions of Part II.B.S.d will be applicable. Liactivation of Certification The permittee may request inactivation of their certification under this permit, once the site has been fully released from their reclamation bond requirements by the Colorado Division of Mining and Geology (CDMG). See Part II.B.S.d of the permit. B. EFFLUENT LIMITATIONS AND CONDTI7ONS -PROCESS WATER DISCHARGE (This section (I.B) only applies to facilities with a discharge of process water.) General Limitations a. There shall be no discharge of sanitary waste waters from toilets or related facilities. b. There shall be no discharge of floating solids or visible foam in other than trace amounts.
