Description of the type of activity resulting in the discharge including the anticipated duration of activity <br />and/or the discharge, anticipated volume, end rate of discharge, end the source of water which is to be <br />discharged; <br />e. Description of any waste water treatment system and recycle/reuse utili7xd; and, <br />f. A map or schematic diagram showing the general area and/or routing of the activity. <br />g. Analysis of the process water to be discharged. <br />h. Description of stormwater discharges, including drainage area. <br />i. Description of present and proposed best management practices at site, including map. <br />j. Any existing water quality data on stormwater discharges. <br />k. For new facilities (beginning operations after October 1, 1992), and for all other facilities applying after <br />July 1, 1993, certification as to the preparation of a SWMP. (Sec Part I.C.6.a of the permit for SWMP <br />certification requirements for existing facilities applying on lime.) <br />4. Aooliration Due Dates <br />At least thirty days prior to the anticipated date of discharge, or by October I, 1992, whichever is later, the owner <br />(or operator if the owner does not operate the facility) of the facility shall submit an application as provided by the <br />Division. <br />Two copies of the completed discharge application form sball be submitted, by certified mail or hand delivery, to: <br />Colorado Department of Health <br />Water Quality Control Division <br />4210 East l Ith Avenue <br />Denver, Colorado 80220 <br />Atteotion: Permits and Enforcement Section <br />As of October 1, 1992, This address will change to: <br />Colorado Department of Health <br />Watet Quality Control Division <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />Attention: Permits and Enforcement Section <br />5. Certification Procedures <br />If the general permit is applicable to the applicant's operation, then a certification will be developed and the applicant <br />will be certified under this general permit. <br />a. Request for Additional Information: The Division shall have up to thirty days after receipt of the above <br />information to request additional data and/or deny the authorization for any particular discharge. Upon receipt <br />of additional information, the Division shall have an additional fifteen days to issue or deny authorvatioo for <br />
