<br />§ 146.70 <br />(a) Quarterly reports to the Direcror comaimng: <br />(q The maximum injection pressure; <br />Q) A description of any event that exceeds op- <br />erating parameters (or annulus pressure or injec- <br />tion pressure as spenfied m the penruc <br />(?) A description of any event which niggers an <br />alarm or shmdown device required pursuant to <br />§ 146.670 and the response taken; <br />(4) The total volume of Fluid injected: <br />(5) Any change in the annular Fluid volume: <br />(6) The physical, chemical and other relevant <br />charactenstics of in)ected Fluids, and <br />(7) The results of monitonng prescribed under <br />§ 146 66. <br />(b) Reporting, within 30 days or with the next <br />quarterly report whichever comes later, the results <br />o(: <br />(I) Periodic tests of mechanical integrity: <br />(?) Any other Iesl of the injection well con- <br />duntd by the penniute i( required by the Direc- <br />tor, and <br />(?) Any well workover. <br />§ 146.70 Information to be evaluated <br />by the Director. <br />This section sell forth the information which <br />muss be evaluated by the Direcror in authonztng <br />Class 1 hazardous waste injection wells. For a new <br />Class I hazardous waste injection well, the owner <br />or operator shall submit all the in(omlalion listed <br />below as pan of the permit application. For an ex- <br />isting or convened Class I hazardous waste injec- <br />tion well, [he owner or operator shall submit all <br />in(ormauon listed below as pan of the permit ap- <br />plication except (or (host items a( information <br />which arc curttm, accurate, and available in [ht <br />exiving permit file. For bosh exerting and new <br />Class I hazardous waste injection wells, certain <br />maps, cross-sections, mbulmions a( wells within <br />the area of review and other data may be included <br />in [ht application by reference provided They are <br />curtent and readily available to the Director (for <br />example, in the permitting agency's files) and su(- <br />ficiemly idemifiable to be retrieved. In casts <br />where EPA issues the permit, all the in(onnation <br />in this section must be submutted to the Adminis- <br />tmtor or his designee. <br />(a) Prior to the issuance of a permit (or an ex- <br />isting Class I hazardous waste injection well to <br />operace or [ht Construction or conversion o(a new <br />Class I hazardous waste m)eclion well, the Dircc- <br />lor shall review the following to assure Thal the re- <br />quiremtn[s of This pan and pan 144 art met: <br />(D Information required in § 14431: <br />(?) A map showing the injection well for which <br />a permit is sought and the applicable area of re- <br />eiew. Within the area of review, the map mull <br />show the number or name and location of all pro- <br />ducing wells, injection wells, abandoned wills, dry <br />:~ <br />holes, surface bodies of wmer, springs, mines (sur- <br />face and subsurface), quarries, water wells and <br />other peninem surface features, including resi- <br />dences and roads. The map should also show <br />(culls, i(known or suspected; <br />(?) A tabulation of all wells wuhin the area of <br />review which penetrate the proposed injection <br />zone or Confining zone Such data shall include a <br />descnption of each well's type, conswction, dale <br />drilled, location, depth, record of plugging and/or <br />completion and any additional information the Di- <br />rector may require; <br />(4) The protocol followed to identify, locale and <br />ascertain the condition of abandoned wells within <br />the area of review which penetrate the injection or <br />the confining zones; <br />(5) Maps and cross-sections indicming the gen- <br />eral vertical and lateral limits of all underground <br />sources of drinking water wuhin the area of re- <br />view, their position relative to the injection (onna~ <br />tton and the direction of water movement, where <br />known, in tach underground source of drinking <br />water which may bt affected by the proposed in- <br />jection: <br />(6) Maps and cross-sections detailing the geo- <br />logic structure of the local area, <br />(7) Maps and cross-sections ill ustmting the re- <br />gional geologic seeing; <br />(B) Proposed operating data; <br />(i) Avemgt and maximum daily rate and vol- <br />ume o(ihe fluid to be injected; and <br />(ii) Average and maxinum injection pressure: <br />(9) Proposed (onnauon testing program to ob- <br />tain an analyss of the chtmicah physical and radi- <br />ological charactensttcs of and other in(ortnation on <br />the injection formation and the confining zone: <br />(10) Proposed stimulation program; <br />(I I) Proposed injection procedure; <br />(12) Schematic or other appropnate drawings of <br />the sudam and subsurface construction derails of <br />the wtlh <br />(13) Conungtncy plans to cope with all shut-ins <br />or well failures so as to prevent migrmion of <br />Fluids into any USDW; <br />(14) Plans (including maps) for meeting mom <br />itoring requirements o(§ 146 68: <br />(IS) For wills within the area of review which <br />penetrate the injection zone or the confining zone <br />but are not properly completed or plugged, the <br />corttctivt action to be taken under § 146.64; <br />(16) Convmction procedures including a ce- <br />menting and casing program, will materials speci- <br />fications and their h(e expectancy, logging proce- <br />dures, deviation checks, and a dolling, testing and <br />Coring program; and <br />(17) A demonstration pursuant to pan 144, sub- <br />par F, thin the applicant has the resources me <br />essary to Close, plug or abandon the will and (or <br />pose-closure care <br />22 <br />
