§ 146.70 (a) Quarterly reports to the Direcror comaimng: (q The maximum injection pressure; Q) A description of any event that exceeds op- erating parameters (or annulus pressure or injec- tion pressure as spenfied m the penruc (?) A description of any event which niggers an alarm or shmdown device required pursuant to § 146.670 and the response taken; (4) The total volume of Fluid injected: (5) Any change in the annular Fluid volume: (6) The physical, chemical and other relevant charactenstics of in)ected Fluids, and (7) The results of monitonng prescribed under § 146 66. (b) Reporting, within 30 days or with the next quarterly report whichever comes later, the results o(: (I) Periodic tests of mechanical integrity: (?) Any other Iesl of the injection well con- duntd by the penniute i( required by the Direc- tor, and (?) Any well workover. § 146.70 Information to be evaluated by the Director. This section sell forth the information which muss be evaluated by the Direcror in authonztng Class 1 hazardous waste injection wells. For a new Class I hazardous waste injection well, the owner or operator shall submit all the in(omlalion listed below as pan of the permit application. For an ex- isting or convened Class I hazardous waste injec- tion well, [he owner or operator shall submit all in(ormauon listed below as pan of the permit ap- plication except (or (host items a( information which arc curttm, accurate, and available in [ht exiving permit file. In casts where EPA issues the permit, all the in(onnation in this section must be submutted to the Adminis- tmtor or his designee. (a) Prior to the issuance of a permit (or an ex- isting Class I hazardous waste injection well to operace or [ht Construction or conversion o(a new Class I hazardous waste m)eclion well, the Dircc- lor shall review the following to assure Thal the re- quiremtn[s of This pan and pan 144 art met: (D Information required in § 14431: (?) A map showing the injection well for which a permit is sought and the applicable area of re- eiew. Within the area of review, the map mull show the number or name and location of all pro- ducing wells, injection wells, abandoned wills, dry :~ holes, surface bodies of wmer, springs, mines (sur- face and subsurface), quarries, water wells and other peninem surface features, including resi- dences and roads. The map should also show (culls, i(known or suspected; (?) A tabulation of all wells wuhin the area of review which penetrate the proposed injection zone or Confining zone Such data shall include a descnption of each well's type, conswction, dale drilled, location, depth, record of plugging and/or completion and any additional information the Di- rector may require; (4) The protocol followed to identify, locale and ascertain the condition of abandoned wells within the area of review which penetrate the injection or the confining zones; (5) Maps and cross-sections indicming the gen- eral vertical and lateral limits of all underground sources of drinking water wuhin the area of re- view, their position relative to the injection (onna~ tton and the direction of water movement, where known, in tach underground source of drinking water which may bt affected by the proposed in- jection: (6) Maps and cross-sections detailing the geo- logic structure of the local area, (7) Maps and cross-sections ill ustmting the re- gional geologic seeing; (B) Proposed operating data; (i) Avemgt and maximum daily rate and vol- ume o(ihe fluid to be injected; and (ii) Average and maxinum injection pressure: (9) Proposed (onnauon testing program to ob- tain an analyss of the chtmicah physical and radi- ological charactensttcs of and other in(ortnation on the injection formation and the confining zone: (10) Proposed stimulation program; (I I) Proposed injection procedure; (12) Schematic or other appropnate drawings of the sudam and subsurface construction derails of the wtlh (13) Conungtncy plans to cope with all shut-ins or well failures so as to prevent migrmion of Fluids into any USDW; (14) Plans (including maps) for meeting mom itoring requirements o(§ 146 68: (IS) For wills within the area of review which penetrate the injection zone or the confining zone but are not properly completed or plugged, the corttctivt action to be taken under § 146.64; (16) Convmction procedures including a ce- menting and casing program, will materials speci- fications and their h(e expectancy, logging proce- dures, deviation checks, and a dolling, testing and Coring program; and (17) A demonstration pursuant to pan 144, sub- par F, thin the applicant has the resources me essary to Close, plug or abandon the will and (or pose-closure care 22
