(g) If an awomatic alarm or shutdown is Ing- gercd, the owner or operator shall immedmtely in- vestigate and identify as expeditiously as possible the cause o(Ihe alarm ar shutoff. If, upon such m- vestigalion, Ihr well appears to be lacking me- chanical integrity, or if monitoring required under paragraph (() of this section otherwise indicmes Thal Iht well may bt lacking mechanical mrcgnly, the owner or operator shall: (I) Cease infection of waste feuds unless au- Ihorized by the Direcor to continue or resume in- jection. (i) Whenever the owner or operator obtains evi- dence that (here may have been a release of in- jected wastes imo an unauthorized zone: (I) The owner or operalar shall immediately CaSC InJCC1100 Of Wuie Ruids, and: (i) Notify the Director within 24 hours of ob- taining such evidence: (ii) Take all ntttssary saps to idemify and charaaeri ze the extern of any release; (iii) Comply wish any remedimion plan speci- fied by the Director; pv) Implement any renxdialion plan approved by the Director; and (v) Where such release is imo a USDW cur rcntly serving as a water supply, place a notice in a newspaper of gtnrml circulation. (2) The Duector may allow the operator to re- sume injection pnor m completing cleanup action if the owner ar operator demonstrates Ihm the in- jection operation will not endanger USDWs. Q) The owner or operator shall notify the Direc- tor and obtain his approval pnor to condoning any will workover. § 146.68 Tes[ing and monitoring re- quirements. Testing and monitonng rcyuircments shall at a minimum include: (a) Monitoring of Ihr injected wastes. (I) The owner or operator shall develop and follow an ap- proved written waste analysis plan (hat describes the procedures to be cartied out to obain a de- railed chemical and physical analysis o(a rep- rcsrntativr sample of the waste, including the qualuy assurance procedures used. AI a minimum, the plan shall specify: (i) The paramenters for which the waste will be analyzed and the rationale for the selection of Ihese parameters; (ii) The test methods (hat will be used to Ies1 (or these paraneters; and (iii) The sampling method Ihai will be used to obtarn a represemarive sample of the waste ro be analyzed. (?) The owner or operator shall repeat the anal- ysis of the injected wastes as described in the waste analysis plan at frequencies specified in the waste analysis plan and when process or operating changes occur that may significantly alter the characrenstics of the waste stream. (3) The owner or operator shall conduct contin- uous or periodic monitoring of selected parameters as regwred by the Director. 2~
