Procedures necessary to assure that pressure <br />imbalances which might cause a backflow or <br />blowout do not occur. <br />(f) The owner or operator shall install and use <br />cominuous recording devices to monitor. the injec- <br />Iion pressure: the (low rate, volume, and tempera- <br />Iure of injected Ruids: and the pressure on the an- <br />nulus between the tubing and the long string cas- <br />ing, and shall install and use: <br />(I) Awomatic alarm and awomatic shut-off sys- <br />tems, designed to sound and shut-in the well whin <br />pressures and flow rates or other parameters ap- <br />proved by the Dircclor exceed a range and/or gra- <br />diem specified in the ptmut: or <br />('_) Automatic alarms, designed to sound when <br />the pressures and ^ow rates or other parameters <br />approved by the Director excced a rate and/or gra- <br />dient specified in the permit, in cases where the <br />owner or operator certifies that a (rained operator <br />wdl bt on-sot at all limes when the well is oper- <br />ating. <br />(g) If an awomatic alarm or shutdown is Ing- <br />gercd, the owner or operator shall immedmtely in- <br />vestigate and identify as expeditiously as possible <br />the cause o(Ihe alarm ar shutoff. If, upon such m- <br />vestigalion, Ihr well appears to be lacking me- <br />chanical integrity, or if monitoring required under <br />paragraph (() of this section otherwise indicmes <br />Thal Iht well may bt lacking mechanical mrcgnly, <br />the owner or operator shall: <br />(I) Cease infection of waste feuds unless au- <br />Ihorized by the Direcor to continue or resume in- <br />jection. <br />(2) Take all necessary seeps Io delennme the <br />presence or absence of a leak: and <br />(3) Notify the Director within 24 hours aher the <br />alaml or shurdown. <br />(h) 1(a loss of mechanical imegrily is discov- <br />ered pursuant to paragraph (g) of this section ar <br />dunng ptnoda mechanical integrity testing, the <br />owner or operator sha1P <br />(D Immediately cease injection of waste fluids: <br />(Z) Take all steps reasonably necessary to deter- <br />mine whether there may have been a rcltaze a( <br />hazardous wastes or hazardous waste constituents <br />into any unauthonzed zone; <br />13) Notify the Director within 24 hours after <br />loss of mechancal mltgnty a dacovered: <br /> <br />(4) Notify the Director whin injection can be <br />txpened to resume; and <br />(5) Restore and demonstrate mechanical imeg- <br />Oly Io the satislaction Of Iht DlrcclOr prior to re- <br />suming injection of waste Rwds. <br />(i) Whenever the owner or operator obtains evi- <br />dence that (here may have been a release of in- <br />jected wastes imo an unauthorized zone: <br />(I) The owner or operalar shall immediately <br />CaSC InJCC1100 Of Wuie Ruids, and: <br />(i) Notify the Director within 24 hours of ob- <br />taining such evidence: <br />(ii) Take all ntttssary saps to idemify and <br />charaaeri ze the extern of any release; <br />(iii) Comply wish any remedimion plan speci- <br />fied by the Director; <br />pv) Implement any renxdialion plan approved <br />by the Director; and <br />(v) Where such release is imo a USDW cur <br />rcntly serving as a water supply, place a notice in <br />a newspaper of gtnrml circulation. <br />(2) The Duector may allow the operator to re- <br />sume injection pnor m completing cleanup action <br />if the owner ar operator demonstrates Ihm the in- <br />jection operation will not endanger USDWs. <br />Q) The owner or operator shall notify the Direc- <br />tor and obtain his approval pnor to condoning any <br />will workover. <br />§ 146.68 Tes[ing and monitoring re- <br />quirements. <br />Testing and monitonng rcyuircments shall at a <br />minimum include: <br />(a) Monitoring of Ihr injected wastes. (I) The <br />owner or operator shall develop and follow an ap- <br />proved written waste analysis plan (hat describes <br />the procedures to be cartied out to obain a de- <br />railed chemical and physical analysis o(a rep- <br />rcsrntativr sample of the waste, including the <br />qualuy assurance procedures used. AI a minimum, <br />the plan shall specify: <br />(i) The paramenters for which the waste will be <br />analyzed and the rationale for the selection of <br />Ihese parameters; <br />(ii) The test methods (hat will be used to Ies1 <br />(or these paraneters; and <br />(iii) The sampling method Ihai will be used to <br />obtarn a represemarive sample of the waste ro be <br />analyzed. <br />(?) The owner or operator shall repeat the anal- <br />ysis of the injected wastes as described in the <br />waste analysis plan at frequencies specified in the <br />waste analysis plan and when process or operating <br />changes occur that may significantly alter the <br />characrenstics of the waste stream. <br />(3) The owner or operator shall conduct contin- <br />uous or periodic monitoring of selected parameters <br />as regwred by the Director. <br />2~ <br />
