<br />§ 146.21 <br />or conversion o(a new Class 1 well Iht Director <br />shall consider the following: <br />(I) Information required in 40 CFR 144.31 and <br />144.31(8); <br />(?) A map showing the injection well(s) (or <br />which a permit is sought and the applicable area <br />of review. Within the area of review, the map <br />must show the number, or name, and lacalion of <br />all producing wells, dry holes, surface bodies of <br />water, spnngs, mines (surface and subsurface), <br />yuamies, wafer wells and other pertinent surface <br />features including residences and roads. The map <br />should also show (cults, if known or suspected <br />Only information of public record is required to be <br />included on This map; <br />(3) A mbulalion of data on all wells within the <br />area of review which penetmle into thr proposed <br />injection zone. Such data shall include a desenp- <br />tion of each well's type, construction, dine drilled, <br />location. depth, record of plugging and/or comple- <br />tion, and any additional infomtmion the Director <br />may require; <br />(4) Maps and cross sections indicating the gen- <br />eral vertical and lamral limits of all underground <br />sources of drinking water within the area of rc- <br />eiew, their position relative to the injection forma- <br />tion and the direction of water movrment. where <br />known, in each underground source of drinking <br />water which may he a(kcted by thr proposed in- <br />jection; <br />(5) Maps and cross sections detailing the geo- <br />logic structure of the local area: <br />(6) Generalized maps and cross sections illus- <br />nmmg the regional geologic setting; <br />17) Proposed operating data: <br />(il Average and maximum daily rate and val- <br />ume of the fluid to be m)ected; <br />(iq Average and maximum injection pressure; <br />and <br />(ii0 Source and an analysis of the chemicah <br />physicah radiological and biological characteristics <br />of injection Quids; <br />B) Proposed formation Testing program to ob- <br />tinn an analysis of the chemicah physical and radi- <br />ological characteristics of and other in(ormmion on <br />tht recaving (onnation; <br />(9) Proposed stimulation program; <br />(10) Proposed injection procedure, <br />(11) Schematic or other appropriate drawings of <br />the surface and subsurface construction details of <br />the well. <br />(12) Comingency plans m cope with all shut-ins <br />or well failures so as to prevent ntigmtion of <br />Quids into any underground source of drinking <br />water; <br />(li) Plans (including maps) (or meeting the <br />monitoring requirements m $ 146.13(6); <br />(14) For wells within the area of review which <br />penetrate the mjenion zone bw are not properly <br /> <br />completed or plugged, the cortective action pro- <br />posrd to be taken undrr 40 CFR 144.55; <br />(IS) Construction procedures including a ce- <br />mrnung and casing program, logging procedures, <br />deviation checks, and a drilling, testing, and coring <br />program; and <br />(16) A certificate that the applicant has assured, <br />Through a performance bond or other appropriate <br />means, The resources necessary to close, plug or <br />abandon the well as required by 40 CFR <br />12?.4?(gl~ <br />(b) Pnor to granting approval (or the operation <br />a(a Class I well the Director shall consider the <br />following information. <br />(U All available lugging and testing program <br />data on the well; <br />(?)
