Part I <br />Page No. 8 <br />Petrrtit No.: CO-0045225 <br />The above reports shall contain the required summarization of the test results for parameters shown in Tables I.B-I and <br />[.B.-2 of the permit. See the permit, for details on such submission. <br />The DMR forms consist of four pages -the top "original' copy, and three attached nocazbon-required copies. After <br />the DMR form has been filled out and signed, the four copies must be separated and distributed az follows: <br />The first ro ipinal signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the <br />following address: <br />Colorado Department of Public Health and the Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the following agency: <br />Li.S. Environmental Protection Agency (8ENF-T) <br />Office of Enforcement, Compliance and Environmental Justice <br />Technical Enforcement Program <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the permittee records. The Discharge Monitoring Report fortes shall be filled out <br />accurately and completely in accordance with requirements of this permit and the instructions on the forms. They shall <br />be signed by an authorized person as identified in Part I.D. <br />4. Additional Notification Requirements: <br />Verbal notification shall be given to the Division within 24 hours of the occurrence of any overflow discharge. A report <br />must be submitted within 5 days aher the end of the discharge. The pettnittee must submit dceumenmtion of the storm <br />event, including the precipitation gauge station (or the pettnittee's own gauge), the estimated volume of runoff <br />produced, the status of infiltration or regeneration, and the steps taken to minimize the amount of overflow. <br />Regenerative releases, which are only authorized for the lower pond at the Ironclad Overburden Storage Area, must <br />also be reported, but tray be reported in a monthly summary, that will be submitted along with the next monthly <br />discharge monitoring report (DMR). The information submitted must include the same az that listed above for overflow <br />discharges, az well az release rates and volumes, and the results of all analyses performed in compliance with section <br />I.B.I. <br />5. Changes In Sources: <br />The permittee is required to visually inspect the base of the waste rock disposal area, the drainage azea downsveam of <br />the tce of the Lower Pond at point 003A, the base of the haul road, the tce of the pond located below the haul road, and <br />the azea contributing flow to the pond at the crusher area. <br />Should any continuous seepage from these areas appear, or if there is visible evidence that uncontrolled sediment <br />discharge into waters of the State is occurring, the permittet is required to notify the Division in an attachment that will <br />be submitted along with the nett monthly discharge monitoring report (DMR). <br />C. DEFIlVIT[ONS OF TERMS <br />'Composite" sample is a minimum of four (4) grab samples collected at equally spaced two (2) hour intervals and <br />proportioned according to flow. <br />
