PARTI <br />Page 5 <br />Permit No. COG-600000 <br />A. COVERAGE UNDER THIS PERMIT <br />3. Aoolication (cont.) <br />g. A sketch of the facility showing all structures, outfalls, and receiving wazets, az well as <br />storage locations of any petroleum or chemicals on site; and <br />b. A chemical analysis of the wazer to be discharged. <br />i. If the discharge is to a storm sewer system, ditch, or other titan made coavryance, approval <br />from the owner of the system must be obtained prior m discharge. In the case of a mobile <br />washing syst®t with multiple discharge locations, the permittce must obtain approval from <br />each owner or municipality for each conveyance system into which the perraittce intends to <br />dischazge. <br />At leazt thirty days prior to the anticipated date of discharge, three copies of the application shall <br />be submitted to: <br />Colorado Department of Public Health and Eavirotttnent <br />WQCD-PE-B2 <br />4300 Cherry Crcek Drive South <br />Denver, Colorado 80222-1530 <br />The Division shall have up to thirty (30) days afar receipt of the above iafotrnation to request <br />additional data and/or deny the authorization for any partiailar discharge. Upon receipt of <br />additional informazion, the Division shall have ao additional thirty (30) days to issue or deny <br />authorizaion for the particulaz discharge. If the applicant does not roceive a request for additional <br />information or a notification of denial from the Division within thirty 30 days, authoriution to <br />discharge in accordance with the conditions of the permit shall be deemed granted. <br />If the Division determines thaz the operation does not fall under the authority of the general <br />permit, then the information ttxeived will be treated as application for an individual permit. In <br />this case, discharge is not allowed until a permit is issued, which may take 180 days. <br />4. Exoiration <br />Authorvation to dischazge under this general ptrrnit shall expire on July 31, 2000. The Division <br />tmtst evaluate and may reissue this general permit once every five years, and must also recertify <br />the applicant's authority to discharge under the general permit az such time. Therefore, a <br />permittee desiring continued coverage under the gweral permit must reapply by January 31, <br />2000. The Division will determine if the applicant may continue to operate under the terms of <br />the general permit. An individual permit will be requited for any facility not reauthorized to <br />discharge under the reissued general permit. For facilities wishing to terminate authorization <br />under the new permit, provisions of Part II.B.S.c. and II.B.S.d. will be applicable. <br />However, for facilities that intend to discharge for periods of less than 3 months, a "Short Tetm <br />Certification" may be issued. For this Short Term Certification, the permittce is covered under <br />the terms and conditions of the general permit for three (3) months and then the certification and <br />the authority to discharge under the cer[ification will expire. If the Short Term Certification is <br />applied, it shall be noted on the certification. <br />
