PART I Page 25 Permit No. COG-F00000 E. REPORTING 1. Reportine of Data Reporting of the data gathered in compliance with Part I.C.1 shall be on a quarterly basis. Reporting of all data gathered shall comply with the requirements of Part I.F. (General Requirements). Monitoring results shall be summazimd for each calendaz gttatter and reported on Division approved discharge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the agencies listed below so they aze received no later than the 28th day of the following month following the and of the quarter (for example, the DMR for the first calendaz gttarur must be received by the Division by April 28th). If no dischazge occurs during the reporting period, 'No Dischazge' shall be reported. The DMR forms consist of four pages -the top "original" copy, and three attached nocarbon- required copies. After the DMR form has been filled out and signed, the four copies mttst be separated and distributed as follows: The fast orieinal signed copy of each discharge motitoring report (DMR) shall be submitted to the Division az the following address: Colorado Department of Public Health and Environment WQCD-PE-B2 4300 Cherry Creek Drive South Denver, Colorado 80222-1530 The fast duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the following agenry: U.S. Environmental Protection Agency Wazer Management Division NPDES Branch SWM-C 999 18th Street, Suite 500 Denver, CO 80202-2466 The third and fourth copies are for the permittce records. The Discharge Monitoring Report forms shall be filled out accurately and completely in accordance with requirements of this permit and the insttuctions on the forms. They shall be signed by an auiltorized person as identified in Part I.E.S. 2. Calculations Calculazions for all limitations which require the averaging of measumnents shall utilia an arithmetic mean unless otherwise specified by the Division in the permit. Do not average is rero for days with no flow. Only average in days in which flow was measured. 3. Sutrolementarv Monitorine [f the permittee, using the approved analytical methods, monitors any parameter more frequently than required by this permit, then the results of such monitoring shall be included in the calculation and reporting of the values roquired in the Discharge Monitoring Report Farm (DMRs) or other forms as required by the Division. Such increased frequency shall also be indicated.
