PART I <br />Page 25 <br />Permit No. COG-F00000 <br />E. REPORTING <br />1. Reportine of Data <br />Reporting of the data gathered in compliance with Part I.C.1 shall be on a quarterly basis. <br />Reporting of all data gathered shall comply with the requirements of Part I.F. (General <br />Requirements). Monitoring results shall be summazimd for each calendaz gttatter and reported <br />on Division approved discharge monitoring report (DMR) forms (EPA form 3320-1). The forms <br />shall be mailed to the agencies listed below so they aze received no later than the 28th day of the <br />following month following the and of the quarter (for example, the DMR for the first calendaz <br />gttarur must be received by the Division by April 28th). If no dischazge occurs during the <br />reporting period, 'No Dischazge' shall be reported. <br />The DMR forms consist of four pages -the top "original" copy, and three attached nocarbon- <br />required copies. After the DMR form has been filled out and signed, the four copies mttst be <br />separated and distributed as follows: <br />The fast orieinal signed copy of each discharge motitoring report (DMR) shall be submitted to <br />the Division az the following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The fast duplicate signed copy of each discharge monitoring report (DMR) shall be submitted <br />to the following agenry: <br />U.S. Environmental Protection Agency <br />Wazer Management Division <br />NPDES Branch SWM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the permittce records. The Discharge Monitoring Report <br />forms shall be filled out accurately and completely in accordance with requirements of this permit <br />and the insttuctions on the forms. They shall be signed by an auiltorized person as identified in <br />Part I.E.S. <br />2. Calculations <br />Calculazions for all limitations which require the averaging of measumnents shall utilia an <br />arithmetic mean unless otherwise specified by the Division in the permit. Do not average is rero <br />for days with no flow. Only average in days in which flow was measured. <br />3. Sutrolementarv Monitorine <br />[f the permittee, using the approved analytical methods, monitors any parameter more frequently <br />than required by this permit, then the results of such monitoring shall be included in the <br />calculation and reporting of the values roquired in the Discharge Monitoring Report Farm (DMRs) <br />or other forms as required by the Division. Such increased frequency shall also be indicated. <br />
