COVERAGE UNDER THIS PERMIT (CONTINUED) <br /> <br />PART I <br />Page 3 <br />PERMIT NO.: COG-500000 <br />h)_ At least thirty (30) days prior.io the anticipated date of <br />discharge, such information-shall be submitted to: <br />Colorado Department of Health <br />Water Quality Control Division <br />4210 East 11th Avenue <br />Denver, Colorado 80220 <br />Attention: Permits and Enforcement <br />The permit issuing authority shall have up to thirty (30) days <br />after receipt of the above information to request additional data <br />and/or deny the authorization for any particular discharge. Upon <br />receipt of additional information the permit issuing authority <br />shall have an additional 30 days to issue or deny authorization for <br />any particular discharge. <br />If the person proposing the discharge does not receive a request <br />for additional information of a notification of denial from the <br />permit issuing authority within the specified time frame, <br />authorization to discharge in accordance with the conditions of the <br />permit shall be deemed granted. <br />If the permit issuing authority determines that the operation does <br />not fall under the authority of the general permit, then the <br />information received will be treated as an individual permit, <br />discharge is not allowed until 180 days after the application has <br />been received. <br />1) Authorization to discharge under this general permit shall commence <br />on October 1, 1987 and shall expire on September 30, 1992. The <br />Division must evaluate this general permit once every five (5) <br />years and must also recertify the applicant's authority to <br />discharge under the general permit, at such time. Therefore, a <br />permittee desiring continued coverage under the general permit must <br />re-apply by April 1, 1992. The Division will determine if the <br />applicant continues to operate under terms of the general permit. <br />
