If the discharge is to a storm sewn system, ditch, or other man made coavryance, approval <br />from the owner of the system rmut be obtained prior m discharge. In the rase of a mobile <br />washing system with multiple dt~tiu ee locatiotu, the pem>ittee must obtain approval from <br />each oava or municipality for each convryance system into whirl the permiree iatwds to <br />discharge. <br />At least thirty days prior to the aatidpazed [late of discharge, three copies of the applicazion shall <br />be submitted to: <br />Colorado Depaztment of Public Health and Envirortmem <br />WQCD-PE-B2 <br />4300 Chary Creek Drive South <br />Denver, Colorado 80222-1530 <br />• <br />The Division shall have up to thirty (30) days after temp[ of the above informazion to request <br />additional data and/or dwy the authorization for ary particular discharge. Upon receipt of <br />additional information, the Division shall have an additional thirty (30) days to issue or deny <br />authorization for the particulaz dischazge. If the applican[ does not receive a request for additional <br />information or a notification of denial from the Division within thirty 30 days, authorizazioa to <br />discharge is accordance with the conditions of the permit shall be deemed granted. <br />If the Division determines thaz the operation does not fall undo the authority of the general <br />permit, then the iaformazion received will be [[eared as appliradon for an individual permit. ht <br />this sax, discharge is not allowed until a permit is issued, whirb may take 180 days. <br />4. iration <br />Authorization to discharge under this general permit shall expire on Jttly 31, 2000. The Division <br />must evaluate and may reissue this general permit once every five years, and must also raertify <br />the applicant's authority m discharge under the general permit az surL time. Therefore, a <br />permittee desiring wntinued coverage under the general permit mmt reapply by Jsntrsry 31, <br />2000. The Division will determine if the applicant may continue to operate under the terms of <br />the general permit. An individual permit will be requirod for soy facility not rauthotized to <br />d'~s*.~ve under the reissued general permit. For facilities wishing to terminate authorization <br />under the new permit, provisions of Part II.B.S.c. and II.B.S.d. will be applicable. <br />However, for facilities that intend to d""ha ve for periods of less than 3 months, a "Short Term <br />Certification" may be issued. For this Short Tam Cerifcaion, the perminee is covead under <br />the terms and conditions of the general permit for three (3) momhs sad then the certification and <br />the authority to discharge under the certification will expire. If the Short Term Certification is <br />applied, it shall be noted on the certification. <br /> <br />
