PARTI <br />Page 14 <br />Permit No. COG-500000 <br />D. M~DNITORING AND REPORTING <br />10. Signatory Reauinmeots <br />a. All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with <br />the following criteria: <br />1) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or <br />her duly wthori7ed representative, if such representative is responsible for the overall operation of the <br />facility from which the discharge described in the form origimtes; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranlang <br />elxb:d official, or other duly suthori7ed employee. <br />b. Changes tn authoriwtion. If an authorintion order paragraph a) of this ejection is no longer accurate because <br />a different individual or position has responsibility for the overall operation of the facility, a new authorvation <br />satisfying the requirements of paragraph a) of this section must be wbmitted to the Division, prior to or together <br />with any relarts, information, or applications to be signed by an authorized representative. <br />c. Certification. Any person signing a document under paragraph a) of this section (except for the SWMP <br />certificatiogl shall metre the following certification: <br />'I certify ua~der penalty of law that this document and all attachments were prepared under my direction <br />or supervision in accordance with a system designed tn assure that qualified persomel properly gather <br />and evaluate. the information submitted. Based on my inquiry of the person or persona who manage the <br />system, or those persons directly responsible for gathering, the information submitted is, to the best of <br />my knowledge end belief, true, accurate, end complete. I am aware that there are significant penalties <br />for submitting false information, including the possibility of fore and imprisonment for knowing <br />viohrtiom.' <br />E. FOOTNOTES -APPLICABLE TO PRECEDING PAGES <br />a/ The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30) <br />consecutive~ay period. Samples shall not be need for more than one (1) reporting; period. <br />b/ The seven (7) day nverage shag be determined by the arithmetic mean of all samples takes in a seven (7) day period. <br />Samples may not be used for more than one (1) reporting period. <br />c/ This limitation shall be determined by a single sample or sU of samples u required by Part I.B.3, Sample Type. <br />d/ Monitoring is required only during periods of discharge. If no discharge occurs, this shall be reported at the specified <br />frequency. (See Part I.D.3.) <br />e! TDS monitoring is only required for facilities discharging in the Colorado River Hasin. TDS may be measured u <br />electrical conductivity where a satisfactory correlation with TDS has been established based upon a minimum of five <br />(5) samples. See the individual Certification Rationale for a determinatioe as to whether monitoring is required. <br />f/ Phosphorus monitoring is required only for facilities which discharge into the following drainage bums: Cherry Creek <br />buin, Chatfield Reservoir downstream of the USGS gage at Waterton and on Plum Creek, Dillon Reservoir basin (i.e., <br />Ten Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek buin. See the <br />individual Certification Rationale for a determination u to whether monitoring ie required. <br />
