10. Signatory Reauinmeots
a. All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with
the following criteria:
1) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or
her duly wthori7ed representative, if such representative is responsible for the overall operation of the
facility from which the discharge described in the form origimtes;
2) In the case of a partnership, by a general partner;
3) In the case of a sole proprietorship, by the proprietor;
4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranlang
elxb:d official, or other duly suthori7ed employee.
b. Changes tn authoriwtion. If an authorintion order paragraph a) of this ejection is no longer accurate because
a different individual or position has responsibility for the overall operation of the facility, a new authorvation
satisfying the requirements of paragraph a) of this section must be wbmitted to the Division, prior to or together
with any relarts, information, or applications to be signed by an authorized representative.
c. Certification. Any person signing a document under paragraph a) of this section (except for the SWMP
certificatiogl shall metre the following certification:
'I certify ua~der penalty of law that this document and all attachments were prepared under my direction
or supervision in accordance with a system designed tn assure that qualified persomel properly gather
and evaluate. the information submitted. Based on my inquiry of the person or persona who manage the
system, or those persons directly responsible for gathering, the information submitted is, to the best of
my knowledge end belief, true, accurate, end complete. I am aware that there are significant penalties
for submitting false information, including the possibility of fore and imprisonment for knowing
viohrtiom.'
E. FOOTNOTES -APPLICABLE TO PRECEDING PAGES
a/ The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30)
consecutive~ay period. Samples shall not be need for more than one (1) reporting; period.
b/ The seven (7) day nverage shag be determined by the arithmetic mean of all samples takes in a seven (7) day period.
Samples may not be used for more than one (1) reporting period.
c/ This limitation shall be determined by a single sample or sU of samples u required by Part I.B.3, Sample Type.
d/ Monitoring is required only during periods of discharge. If no discharge occurs, this shall be reported at the specified
frequency. (See Part I.D.3.)
e! TDS monitoring is only required for facilities discharging in the Colorado River Hasin. TDS may be measured u
electrical conductivity where a satisfactory correlation with TDS has been established based upon a minimum of five
(5) samples. See the individual Certification Rationale for a determinatioe as to whether monitoring is required.
f/ Phosphorus monitoring is required only for facilities which discharge into the following drainage bums: Cherry Creek
buin, Chatfield Reservoir downstream of the USGS gage at Waterton and on Plum Creek, Dillon Reservoir basin (i.e.,
Ten Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek buin. See the
individual Certification Rationale for a determination u to whether monitoring ie required.
