The Division reserves the right W require additioml information in the report, on a <br />case-bycase basis, ae needed. <br />All reports required for submittal shall be signed and certified for accuracy by the permittee (see Part I.D.10). <br />A signed copy of the above report forms shall be submitted to the following address: <br />Colorado Department of Health <br />Water Quality Control Division <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Deaver, Colorado 80222-1530 <br />Attention: Permits and Enforcement Section <br />5. Test Procedures <br />Test procedures for the analysis of pollutants shall conform to regulations published pursuant to Section 304 (h) of the <br />Clean Water Act, and Colorado State Effluent Limitations (10.1.x, under which such procedures may be required. <br />Rernrding of Results <br />For each ^wa~+~rement or sample taken pursuant to the requirements of this permit, the permittee shall record the <br />following information: <br />a. The exact place, date, and time of sampling; <br />b. The dates the amlyses were performed; <br />c. The person(s) who performed the sampling; <br />d. The person(s) who performed the analyses; <br />e. The .natyucel techniques or methods used; and <br />E The results of all required analyses. <br />Additional Monitoring by Permittee <br />If the permittee monitors my pollutant u the location(s) designated herein [tare frequently then required by this permit, <br />using approved analytical methods as specified above, the results of such monitoring shall be included in the calculation <br />and repotting of the values required in the Discharge Monitoring Report Form or other forms as required by the <br />Division. Such increased frequency shall also be indicated. <br />8. Records Retention <br />All records apd information resulting from the moniaring activities required by this permit including all records of <br />analyses performed and calibration and maintemnce of instrttmentation and recordings from continuous monitoring <br />instrumentation shall be retained for a minimum of thrx (3) years, or longer if requested by the Regioml <br />Administrator m the State Wa[er Quality Control Division. <br />9. Reporting to Muniapality <br />Any permitted facility discharging to a municipal storm sewer shall provide the municipality with a copy of the permit <br />application, upon request. A copy of the SWMP shall also 6e provided to the municipality upop request. <br />
