(See Part I.D.10 of the permit.) e. Quarterly Report - Process Water For facilities allowed to discharge process water, reporting of the data gathered in wmplimce with Part I.B.3.a shall be on a quarterly basis. Monitoring results obtained during the previous 3 months shall be summarized and reported on Division approved Discharge Monitoring Report forms (DMRs), postmarked no later then the 26th day of the month following the completed calendar quarter. For example, for the 1st quarter (Jmuary, February, and March) the quarterly report moat be received at this office postmarked no later then April 26. If no discharge occurs during the reporting period, 'No Discharge' shall be reported. b. Annual Report - Process Water For facilities not allowed to discharge process water (see Part I.B.3.b of the permit), a report on whether or not a discharge of process water has takes place at Ore facility is required, in letter form, on m amual basis. The report shall rover October 1 through September 30 of each year, and will be due to the Division by the following Otxober 28, beginning with the fast year or partial year of permit coverage. The Division reserves the right the require additional information in the report, on a teen-by~ase basis, ae needed. 4. Rerartin¢ of Data - Stormwatrr Discharners a. Amual Report - SWMP The permittee will be required to submit m amual report, covering Jmuary 1 through December 31 of each year, w the overall compliance with the SWMP. The amual report will contain, at a ptinimum: 1) Name of permittee, address, phone number, and permit certification number. 2) A report on the facility's overall complimce with the SWMP. 3) A summary of each comprehensive stormwater facility inspection made, including date, findings, and action taken. 4) Results and interpretation of my stormwater monitoring performed. The amual report will be due W the Division on or before February 15 of each year, after the fast full year of coverage under the permit. The exact due date for the permittee's fast amual report will be listed in their permit certification. In no case will m amual report be due before February 15, 1994, unless specifically
