The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />PB1. Rip-rap is not properly aligned with the diversion Immediately <br />channel along the north side of Pond UB2. PB or PV PB <br /> TOPIC(S) SP <br />Corrective Action: The channel needs to be adequately <br />re-routed into Pond UB2 and properly armored with rip- <br />rap as soon as possible. <br />PV1. a) Culvert diameters for Ponds UB2, Nffi1, LB1 and LB3 Immediately <br />measured in the field are smaller than designed. PB or PV PV <br />b) The emergency spillways for UB1, UB2, MB1, LB1 and TOPIC(S) SC <br />LB3 were found to be undersized relative to the typical <br />minimum spillway profile. <br />c) A structure has not been installed near the entrance <br />gate to divert runoff on the paved access road into the <br />entrance gate sediment pond. <br />Corrective Actions: As soon as possible: <br />a) Either install the appropriate sized culverts or <br />demonstrate to the Division's satisfaction that the <br />existing ones will be adequate for the design <br />hydrograph. <br />b) If a new runoff model based on the existing culverts <br />is submitted, the Operator will need to submit for <br />Division approval a re-evaluation of the minimum <br />spillway profile including any re-sizing which may be <br />necessary. Reconstruct spillways to meet the revised <br />minimum spillway profile as approved by the Division. <br />c) Install an entrance gate area diversion structure <br />acceptable to the Division. <br />PB2. The southern embankment of the entrance gate pond Immediately <br />embankment is wet and soft.
