r _4_ <br />NOTICE PROCEDURES <br />After the application is considered "filed" (pursuant to RULE l.Z(10) with the Mined <br />land Reclamation Division, notice procedures outlined in RULE 3.2 and/or 4.2 must be <br />followed. (Format for such notices will be included in the letter yov received from <br />i , the MLR staff informing you of the Completeness ("filing") of your application. <br />FINANCIAL WARRANTY <br />' Prior to permit issuance, proper performance and financial warranty must be received <br />in accordance with RULE 7. Please obtain the appropriate form for your chosen method <br />of financial warranty. <br />' COLORADO WATER LAWS <br />The permittee is encrovraged to contact the State Engineer, with regard to the <br />~ ' administration of water rights, and the Colorado Department of Xealth, with regard to <br />the discharge of pollutants into the waters of the state. Violation of Colorado <br />water Zaws could result in enforcement actions taken by the State Engineer and/or the <br />Colorado Department of Xealth. <br />ADDITIONAL INFORMATION <br />' This space can be referenced for further information of any of the items on this form. <br />The proposed permit boundary is within an approximate 30 acre area previously <br />' disturbed by dry alluvial sand and gravel operations. Last year approximately <br />' 15 acres of previously disturbed lands were reclaimed. Within this proposed <br />permit boundary approximately 20,000 cubic yards of gravel remain in two <br />mounds.
